Position Summary
The Referral Specialist primarily performs clerical and clinical management functions including, but not limited to appropriate patient information coordination, timely tracking and follows up of referrals generated by referring providers and receipt of consult from specialist, daily provider notification of departmental referral status.
Essential Functions
- Completes, monitors, and tracks specialty referrals for patients as specified by the primary care provider and/or specialist to ensure the delivery of quality and timely patient care.
- Daily maintenance and data entry into the EMR regarding the departmental status of all referrals.
- Ensures that all referrals generated contain:
- Initiation date of the referral
- Referring provider
- Clinical reason and purpose
- Urgency of referrals
- Expected timely receipt of referrals
- Provides notification of referrals status
- Reminds patients who are self-referrals to bring in all documents relevant to their visit with outside specialist to visit with their PCP.
- Prepares a monthly referral productivity report that include
- Number of referrals processed
- Number of appointments scheduled and kept
- Number of appointments rescheduled
- Number of specialty reports received
- Number of outstanding reports for the reporting period
- Completes monthly submission of all "Referral Tracking Logs".
- Assists both providers and patients in the selection of an appropriate specialist.
- Performs other duties as assigned
