Qualifications and Experience
- Graduate of a school of social work approved by the Council of Social Work Education with a master’s degree (MSW) or has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and will be supervised by an MSW. (Diploma or transcripts to be provided to agency)
- If education level is baccalaureate then employee must have one year of experience in a healthcare setting.
- Prior Home Care experience preferred but not necessary.
- Demonstrate experience in working with patient/families.
- Demonstrates good verbal and written communication and organizational skills.
- Licensure by the state.
- Must maintain current drivers license, auto insurance, CPR card and TB test on file.
- A criminal background check must be performed before hire.
- Complete and pass skill evaluation test.
Essential Duties and Responsibilities *
- Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical and progress notes on all patients referred to social work.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
- Works with family as needed.
- Consults and coordinates services with other disciplines.
- Provides information to patients or families/caregivers and community agencies.
- Serves as liaison between patients or families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Maintains and develops contacts with public and private agencies as resources for patient and organization personnel.
- Participates in; the development of the total plan of care in consultation with the physician and other care team member and case conferences as required.
- Assists physician and other team members in understanding significant social and emotional factors related to health problems.
- Participates in discharge planning.
- Participates in in-servicing programs.
- Other duties as delegated by the Clinical Director/Supervisor.
