Evaluates chronically mentally ill childrens needs and determines or refers them to appropriate resources in order to best integrate the child into the community

 

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Maintains caseload in compliance with DBH and State standards. Completes registration process for new children within ten days of referral from DBH. Completes all necessary documentation within required timeframes.
  • Maintains a minimum of 75% productivity on a weekly basis.
  • Identifies community resources and facilitates linkages with psychiatric, social, vocational, medical, housing, recreational, forensic, and drug/alcohol systems for the coordination of services to meet individual and family needs.
  • Engages childs support network including family members and providers to assist in determining appropriate recommendations for continuity of care.
  • Maintains contact with each child every 14 days minimally as well as in home face-to-face contacts every 30 days. Monitors individual progress in the community through ongoing communication and follow-up with supports.
  • Participates in regular team meetings and discusses caseload on a regular basis to keep the team and supervisor updated.
  • Facilitates involuntary and voluntary hospitalization processes according to established procedures. Liaisons with Mental Health delegates and/or CRC services. Visits hospitalized children weekly and is involved in discharge planning with hospital staff.
  • Updates all chart documentation to ensure accuracy in accordance with the State and DBH regulations.
  • Meets paperwork deadlines as established in the unit. Accurately completes documentation of actual calls, contact attempted and face to face visits within the timeframes and processes defined by DBH, Agency and unit standards.
  • Maintains cash receipts for expense reports. Completes and submits expense reports monthly.
  • Processes requests for client funds within the guidelines of Consortium policy. Keeps track of funds spent on clients behalf and maintains receipts to ensure appropriate reporting.
  • Follows all unit and agency guidelines for using agency vehicles.
  • Makes sound decisions regarding managing crisis situations and personal safety.
  • Ensures pagers and cell phones are charged and in working condition at all times. Responds to calls within 15 minutes.

Education and/or Experience

Bachelor's degree (B. A.) from four-year college or university in an acceptable discipline and one (1) year relevant experience.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Computer Skills

To perform this job successfully, an individual should have knowledge of Internet Explorer and Internet based training and evaluation tools and MS Word word processing software. Basic knowledge of MS Excel helpful.

Certificates, Licenses, Registrations

CPR certification. Must have a valid drivers license and an acceptable driving record.
