Definition and Role
The Clinical Educator/Trainer is an LPN who supports the Nursing Home Program by onboarding, training, and remediating staff in the positions of Licensed Practical Nurse (LPN), Wound Medicine Technician (WMT), and Wound Medicine Assistant (WMA). This role ensures staff competency in wound care procedures, EMR use, and administrative workflows, while promoting MedCentris’ values, vision, and commitment to quality patient care.
Qualifications:
- Graduate of accredited School of Nursing
- Current licensure with the State Board of practical nurse examiners with no restrictions
- Minimum two to three years of experience working in a medical office or health care facility
- Membership in professional and or community organizations desirable
- Ability to communicate verbally and in writing effectively
- Demonstrates proven decision-making skills
- Strong knowledge and understanding of medical terminology, wound care, strong clinical and interpersonal skills
- Networking experience within the medical industry beneficial
- Evidence of strong knowledge of Microsoft Excel, Word, PowerPoint and Computer skills
- Knowledge of communication processes and organization
- Commitment to accuracy and attention to detail
- Must be able to travel possibly overnight four days per week
- CPR/BLS required
- Nursing Home Experience Preferred
Training & Education
- Provide onboarding and ongoing wound care and EMR education to Nursing Home Program staff.
- Implement training MedCentris staff to perform all duties outlined in the LPN, WMT, and WMA job descriptions.
- Travel up to 4 days per week to provide training, remediate existing staff, &/or educate the Nursing Home Staff.
- Deliver instruction on multitasking, care coordination, and teamwork principles.
- Maintain competency in all clinical and administrative duties taught.
Clinical Support
- Perform nursing procedures in direct patient care under the supervision of an MD/NP/PA.
- Demonstrate and instruct wound care techniques, EMR documentation, and patient care best practices.
Administrative Training
- Provide education on basic administrative tasks, including filing, copying, faxing, data entry, and document management.
- Train staff in appointment scheduling, care coordination, and communication with team members and facility staff.
Collaboration & Liaison
- Establish and maintain rapport with long-term care (LTC) facility leadership.
- Act as a liaison between LTC facilities and MedCentris staff to promote effective communication and problem resolution.

A Licensed Practical Nurse (LPN) performs nursing procedures in direct patient care under the supervision of an MD NP or PA. Must have a good working knowledge of basic office equipment and good phone and computer skills. They are expected to multitask and adequately solve problems while providing quality service and maintaining a positive relationship with the Executive team Nurse Practitioners, Physicians, Physician Assistants, Health care providers administrative staff and Partnering agencies. It is also important that the LPN follows all rules related to patient and employee confidentiality.
Licensed Practical Nurse (LPN) – Job Responsibilities and Duties
Clinical Care
- Observe, record, and report to supervising MD/NP/PA the patient’s general physical and mental condition and note any changes in status.
- Administer prescribed medications and treatments in accordance with the treatment plan, state, and local regulations.
- Assist MD/NP/PA with specialized procedures and wound care, including advanced dressings (e.g., TC, dermal skin substitutes, compression wraps) if competency is documented.
- Perform wound photo documentation, wound measurements, and assessments.
- Perform routine venipuncture if competency is documented in the personnel record.
- Notify provider or charge nurse of concerns identified in the patient’s living environment (hospital, LTAC, nursing home, or home).
- Provide patient/family teaching and counseling per provider-developed plan.
- Assist with patient testing, specimen collection (cultures, PCRs, labs, biopsies), labeling, processing, and shipping.
Documentation & Communication
- Complete all documentation at the facility or immediately after patient care rounds, including EMR entries and orders.
- Communicate documentation completion with the Clinical Coordinator.
- Maintain patient confidentiality in verbal, written, and electronic communication.
- Participate in team meetings to discuss patient progress, treatment plans, and overall health status.
Operational & Administrative
- Assist in obtaining all required consents and upload to EMR.
- Assist in obtaining all tests and lab results and upload to EMR.
- Support appointment scheduling, confirmation, and coordination with the Care Coordinator.
- Provide updates on patient status, wound progress, satisfaction, and disposition.
- Maintain inventory and order supplies as directed by leadership or provider.
- Travel as required to various clinic locations, home visits, or nursing home programs.
Professional Standards
- Adhere to OSHA infection control and bloodborne pathogen standards.
- Follow dress code (no acrylic nails; nails trimmed and clean; wear ID badge).
- Maintain professional conduct, timely attendance, and compliance with company policies.
- Complete competency validations and participate in continuing education.
A Wound Medicine Technician provides for the delivery of patient care; works collaboratively and under the direct supervision of a licensed nurse or MD/NP/PA; Communicates effectively with coworkers, patients, family, and visitors; Provides a safe environment for patients; Performs all functions as delegated by the lead nurse/MD/NP/PA and keeps nursing personnel informed and adheres to the policies and procedures of MedCentris and the partnering facility.
Wound Medicine Technician/Assistant (WMT/WMA) – Job Responsibilities and Duties
Clinical Support
- Remove dressings, cleanse wounds, perform photography, take measurements, and obtain vital signs.
- Prepare and clean patient treatment areas before and after care.
- Clean, set up, and sterilize instruments for procedures.
- Apply dressings as directed by licensed personnel.
- Assist with patient testing, treatments, and specimen collection (cultures, PCRs, labs, biopsies), including labeling and shipping.
- Assist patients with safe transport via wheelchair or stretcher.
Documentation & Communication
- Complete required documentation at the facility or immediately after patient rounds.
- Assist in obtaining consents and uploading them to the EMR.
- Communicate effectively with providers, staff, patients, and families.
- Provide updates on patient status, wound progress, satisfaction, and disposition.
- Communicate documentation completion with the Clinical Coordinator.
- Maintain patient confidentiality in verbal, written, and electronic communication.
Operational & Administrative
- Maintain and order supplies as directed; report office/medical supply needs.
- Assist in obtaining all tests and lab results and upload to EMR.
- Support appointment scheduling, confirmation, and Care Coordinator communication.
- Perform administrative tasks (faxing, DME orders, data entry, ER patient status updates, email/Tiger Connect checks).
- Assist provider and staff with equipment setup, report defects promptly.
- Autoclave surgical instruments.
