What You’ll Do:

  • Conduct comprehensive MDS assessments and ensure compliance with federal and state regulations.
  • Collaborate with interdisciplinary teams to create and update individualized care plans for residents based on assessments and resident needs.
  • Train and support nursing and rehab staff in MDS processes and documentation.
  •  Review and analyze data to maintain quality assurance and improvement in resident care.
  • Participate in care conferences with residents and families to discuss care plans and goals.

Qualifications

  • Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Washington.
  • Experience in long-term care and knowledge of the MDS process is preferred.
  • Strong assessment, organizational, and communication skills.
  • Ability to work effectively in a team-oriented environment and foster positive relationships with residents, families, and staff.
  • Certification in MDS (e.g., MDS Coordinator certification) is a plus.
  • Must be able to pass a background check and meet all state requirements
