Values Into Action, a leader in person-centered and community-focused services in partnership with people with disabilities, is looking for a determined and energetic Community Support Facilitator to work throughout Philadelphia and Delaware County.
Values into Action supports adults with intellectual and developmental disabilities one person at a time, on their own terms, and exclusively in their own homes and communities. As a Community Support Facilitator, you will provide community-based leadership to ensure high quality customer service and overall support services. You will provide the service of supervision and mentoring to Support Partners so that the health, safety and rights of the people who accept support with Values Into Action are assured.
Some responsibilities will include the following:
- Assisting the person in realizing their full civil rights as a participating member of their community and as a citizen of the United States.
- Promote the safety and welfare of the person by offering opportunities for healthy choices, responding to emergencies in a quick and effective manner, providing First Aid should the occasion arise and helping to obtain prompt medical and/or psychiatric attention from a qualified professional when necessary.
- Immediately reporting suspected or observed abuse and neglect.
- Support the organization’s mission, as well as the person’s needs and desires and team’s goals.
- Work collaboratively with members of the Residential Leadership Team
- Be knowledgeable about regulatory and service system standards.
- Provide On-Call & Back-Up Support, including direct service to those accepting support with Values Into Action, as needed.
- Provide the service of supervision, consistent with the organization’s mission, vision, values and practices.
- Maintain a healthy and effective relationship with the person supported as well as their teams.
- Support the person to lead the team process to the extent they are willing and able, including active participation in Individual Service Plan (ISP) meetings, and staffing processes.
- Hear and respect the person’s communication modality and support opportunities for additional communication supports, when applicable.
- Support the person’s self-sufficiency by using independent living technology, as needed and requested by the person.
- Mentor Support Partners in helping people using our services to build and/or sustain natural support systems such as friendships.
- Provide constructive support, to Support Partners as well as the person supported, in a manner consistent with the person’s preferences, values and culture, and model these for the team.
- Participate in the staffing process, including identifying and implementing appropriate staffing schedules, communicating recruitment needs, and facilitating interviews.
- Participate in the reconciliation of employee time and attendance and service documentation and approve employee timecards for payroll.
- Ensure the person and their Support Partners are collaboratively working toward ISP outcome goals.
- Attend and participate in all meetings and training sessions as scheduled.
- Maintain personal training and qualification standards per our funders
- Follow all organizational guidelines as they currently exist and participate in the development and implementation of future goals, guidelines and solutions.
- Perform other duties as requested or assigned by the person, their family, or the organization’s leadership.
