Position Summary
The Wellness Nurse provides wellness services to independent living residents
ESSENTIAL DUTIES AND RESPONSIBILITIES (*CF indicates Critical Function)
- In conjunction with Director of Resident Services, assesses prospective independent living residents prior to their acceptance to the independent living community (*CF)
- Provides assistance with understanding medications, immunizations, blood pressure readings, and test results
- Assists in locating health care professionals such as medical specialists, physical therapists, private nurses, home care agencies, and medical equipment suppliers
- Assists Director of Resident Services in performing annual, and more often as needed, check-ins with all Independent Living residents
- Evaluates residents and makes recommendations for those residents that can benefit from physical therapy, occupational therapy, and/or speech language pathology (including cognitive therapies)
- Provides basic first aid for emergencies only
- Takes blood pressures, pulse, and temperature of residents as needed
- Assists the Director of Resident Services with Independent Living residents’ transitions to the Health Center; assessments of residents, communication with physicians and Health Center staff, meetings with family members
- Instructs and counsels residents regarding health maintenance and compliance with prescribed therapeutic regimens
- Maintains continuous contact with resident or next kin when a resident is hospitalized, and provides updates to the appropriate members of leadership
- Ensures resident emergency packets are up to date and complete
- Maintains line lists for flu, RSV, COVID, Norovirus, viral pneumonia, and any other communicable disease as assigned
- Maintains a record of and reports on resident accidents and incidents at monthly Safety Committee meeting
- Acts as a liaison to Independent Living for all tasks under the infection control umbrella, guided by the WP Infection Control nurse. Communicates related changes in process or protocols effectively to community members
- Completes all required training and in-services
- All other duties as assigned
GENERAL SKILLS AND ABILITIES
- Maintains high standards for work areas, attitude and appearance
- Understands that honesty and ethics are essential in the performance of duties
- Possesses knowledge and skills necessary to provide services to and communicate with primarily elderly population
- Has knowledge of the principles of growth and development over the life span
- Able to assess data reflecting of the resident’s status and interpret the appropriate information needed identify each resident’s requirements relative to his/her age-specific needs
- Maintains predictable and reliable attendance
Qualifications
- Graduate of an accredited school of nursing; RN with two years experience or LPN with five years of experience
- Two years of nursing with the geriatric population preferred
- One year of supervisory experience preferred
- Holds a current, valid nursing license to practice in NY State
- Working knowledge of Microsoft Office programs
- Excellent communication skills, both written and oral
