Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966), Outer Cape Health has a long history of successful growth and expansion in pursuit of this mission. A day in the life of this role:
The Nurse Practitioner (NP) / Physician Assistant (PA) at Outer Cape Health Services ("OCHS") provides comprehensive and continuing care to patients requiring routine and emergency treatment at health centers or other locations within OCHS's scope of practice. The NP/PA assesses, diagnoses, and treats episodic illnesses, and manages chronic health problems of adult and pediatric patients to ensure a positive patient experience and positive medical outcomes.Â

Summary of duties:
- Assess patient wellness/illness by obtaining a complete medical history, ensuring that findings are properly recorded in the patient's medical record.
- Perform physical examinations, order laboratory tests, and analyze results to evaluate acute and chronic complaints.
- Diagnose illness or health needs based on analysis and evaluation of clinical data collected; plan and provide treatment according to protocols and/or in consultation with the supervising physician and/or Medical Director.
- Manage care for patients across specialties such as pediatrics, gynecology, internal medicine, family practice, and family planning.
- Provide consultation to nurses who triage patient telephone calls and walk-ins; refer patients for specialty services after consultation with a physician.
- Collaborate with others to plan, develop, and evaluate new patient care and community health programs and policies; participate in OCHS quality assurance activities.
- Maintain and enhance professional knowledge of theory and practice by attending CME conferences, educational programs, and seminars as per licensure requirements.
- Perform minor surgical procedures under the supervision of a physician, as required.
- Track patient care through follow-up visits and ongoing management.
- Participate in the on-call schedule rotation, requiring night and weekend hours.
- Adhere to OCHS policies and procedures.
- Perform other related duties and projects as required to meet expectations of the position.
