Location: Denver, CO
Schedule: * Split time between both Denver Facilites
Mon 10am-6:30pm
Tues-Friday 8am-4:30pm
2nd Saturday of every month 9am-1:30pm

Position Summary for the Registered Nurse ( RN) : Under general supervision, adheres to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare, consumer safety, and quality outcomes. Assists with providing direct patient care for those disproportionately at risk for acquiring or living with HIV/HCV.
Essential Job Functions of the Registered Nurse:
- Provides direct care coordination services for patients
- Screens patients and arranges initial appointments
- Leads client intake process including reviewing the program, ensuring completion of enrollment paperwork
- Provides patient education (e.g. risks and benefits of Suboxone or Vivitrol, symptom management/withdrawal, understanding pain)
- Thoroughly performs focused assessments and documents patient’s signs and symptoms
- Accurately and appropriately documents client behaviors and self-reports
- Monitors lab tests, e.g. urine toxicity, blood tests
- Assists with patients’ stressors and resources as they arise
- Obtains prior authorizations and coordinates with pharmacies for medications
- Enhances patient follow-up and program adherence
- Consults with prescribers and behavioral health providers
- Communicates patient status changes to the Care Team – teaming with provider, primary care provider, behavioral health provider, nurse, pharmacists, and other internal and external supports on patient’s Care Team
- Completes external referrals for medical needs
- Maintains DEA list of active patients
- Prepare patients for, and assist with, examinations and treatments
- Consult and coordinate with health care team members to assess, plan, implement, and evaluate patient care
- Observe/record patient behavior and performs initial health screening and diagnostic exams
- Monitor, record, and report symptoms and changes in patient’s
- Complete other duties as assigned as the onsite Registered Nurse
