Objective:
TCN Behavioral Health Services, Inc., a Non-Profit Organization, is seeking a qualified applicant for the position of Emergency Services Care Coordinator.

Fairborn, OH

Full-time

Some evenings and weekends may be required
 

Primary Responsibilities:

  • Serves as the agency representative on the Quick Response Team (QRT), working on a team following up on recent overdoses by referring client to counseling and other supportive services. 
  • Provides clinically competent substance use treatment services.
  • Develops clinical impressions.
  • Formulates and reviews treatment plans.
  • Plans and conducts individual and group counseling sessions in accordance with identified treatment goals and objectives.
  • Participates in drug screen monitoring.
  • Develops and maintains quality, accurate clinical charts for all clients served. 
  • Jail Assessment and Screening Services in the Greene County jails
  • Facilitating initial appointments with appropriate agencies along with arranging transportation.

Qualifications

Minimum Qualifications: 

  • Education: Chemical Dependency and/or Social Work Licensure preferred; minimum of a bachelor's degree in social work or behavioral health. 

  • Certification: CPR; First Aid 

  • Other: Work requires transporting clients to and from appointments related to treatment needs, Ohio Driver’s License in good standing is required. Must have reliable transportation and a cellular phone.
