Cohen Fellows will join in the mission of providing evidence-based therapy to individuals, couples and families. The Steven A. Cohen Military Family Clinic has a commitment to training, consultation and proficiency in Cognitive Behavioral Therapy (CBT), Cognitive Processing Therapy (CPT), and Prolonged Exposure (PE).
The Intake Coordinator provides support to the Cohen Clinic as the first contact for services. This position will conduct phone and face to face screenings and intakes on-site, via telehealth, and off-site (if applicable) for veterans, military, and their families. The Intake Coordinator will work with a multi-disciplinary team for case assignments, consultations, and care coordination.
Compensation Range:
The base pay for this position is $68,000 - $80,000 annually.
Responsibilities:
- Conduct initial intakes with veterans and /or family members. Coordinate case assignment with Cohen Clinic multi-disciplinary team. Document detailed intakes on the electronic health records (EHR) system; maintain comprehensive client referral tracking system. Prepare reports as requested by Clinic Director and Outreach Director.
- Provide services in-person and/or via telehealth platform, as required.
- Meet regularly with Lead Clinician or Clinic Director to coordinate case assignments, Outreach Manager, and Case Manager for client activities within clinic community room and at events, report referral to admission conversion rate; Provide ongoing program evaluation and recommendations to the Clinic Director and Outreach Manager for continuous growth and quality.
- Knowledge of local community resources and provide warm hand-offs for veterans and families needing external referrals.
- Carry caseload of 5-10 clients or based on clinic needs.
