Center for Human Services (CHS) is seeking a full-time, skilled, organized and caring outreach case manager with experience working with unhoused individuals with substance abuse and mental health issues. This position involves working as part of an interdisciplinary outreach team in the community. This is a full-time position.
POSITION SUMMARY: Requires in the field substance abuse screening, case management, and referral. Embedded and co-located member of Community Assessment, Response & Engagement (CARE) Multidisciplinary Team (MDT).
DUTIES:
- Conduct outreach and engagement activities in the community.
- Meet individually with clients regularly previous to substance use assessment to develop trust, educate, discuss substance use and consequences, discuss treatment options, information and resources, and motivate pre-contemplative clients toward treatment for SUD.
- Provide crisis intervention services as indicated by client need during outreach and engagement, recognizing and intervening appropriately given signs and symptoms of use and withdrawal and/or mental health decompensation.
- Complete substance use screenings of adults and arrange for substance use disorder services.
- Provide case management, including locating and transporting clients to appointments, arranging emergency housing and recovery residence, picking up medications and other ancillary services to support getting client in to treatment.
- Collect and organize program data.
- Maintain case records in compliance with contractual, accreditation, professional ethics and licensing requirements.
- Make daily decisions in assessing case needs, planning, and facilitating treatment and make joint service team decisions.
- Utilize motivational interviewing, stages of change theory, and state-wise treatment strategies to intervene and motivate clients for change.
- Collaborate with fellow CARE MDT members.
- Maintain productive relationships with community partners, including Modesto Police Department Community Health and Assistance Team (CHAT).
