SUMMARY/OBJECTIVE:
Direct Support Professional – Self Direction provides individualized services and supports to people who receive Self-directed services through their budget.
Essential Functions (Job Duties)
1. Provide person focused supports to people as outlined in his/her Life Plan.
2. Participate in the development of the Habilitation Plan and Valued outcomes as written by the Broker.
3. Provide support for Valued Outcomes as written by the Broker in the Staff action plan.
4. Participate in circle of support meetings. Promote teamwork by maintaining open and positive communication.
5. Use the Code of Ethics and Core Competencies at all times while supporting each person, interacting with the public, and communicating with a person’s family or Circle of Support.
6. Assist people with all aspects of personal care using appropriate level of privacy.
7. Assist people with daily living skills.
8. Assist with lifting, transferring and positioning when necessary.
9. Communicate information to Broker and Fiscal Intermediary as needed.
10. Ensure safety of people by providing the prescribed level of supervision and safeguards at all times.
11. Contemporaneously and accurately, document service delivery as per policy and procedure manual.
12. Maintain expected standards of productivity.
13. Assist with ensuring the person is able to manage self-directed budget.
14. Demonstrate flexibility with scheduling.
15. Protect each person’s confidentiality.
16. Participate in required trainings.
17. Professionally represent Citizen Advocates, Inc. with families, other organizations and the community as a whole.
18. Follow program policy and procedure manual as written.

This is a full-time/part time/relief position.

Hours and schedules may vary depending on Company need and workload. Employees will be given as much advance notice as possible when such changes occur.
