Description

Comunliife is looking for an experienced Director of Performance and Quality Improvement to join their Comunilife’s Quality Improvement Unit. The Director of Performance and Quality Improvement is a senior-level professional who oversees performance outcomes, quality assurance, and improvement activities of Comunilife programs.The Director of Quality Improvement is responsible for developing and implementing program policies, standards, and systems that ensure regulatory compliance and excellent service delivery.  The Director of Performance and Quality Improvement oversees the Quality Improvement unit ensuring quality assurance processes, tracking and monitoring processes and outcomes are well managed and consistent producing regular outcome reports driving improvement. The Director identifies non-compliance issues, recommends and implements improvements to systems including program management input.The Director oversees agency wide incident management and administrative follow up.  Identifies incident trends and recommends and implements QI strategies.Overall, the Director of Performance and QI develops and implements policies, standards, and systems that ensure consumer satisfaction, regulatory compliance, and operational excellence.

Essential Duties:
 
  • Manage and supervise QI Unit, including supervising the Quality Assurance Coordinator and Data Analyst.
  • Oversee agency-wide QI, QA and data analysis activities and projects.
  • Improve and streamline policies, procedures and efficiencies.
  • Identify and reduce agency wide program audit and regulatory risk.
  • Be well versed in relevant OMH, HRA HASA, HRA DOHMH, HPD and OTDA guidelines, standards and policies as well as HOPWA, OMH 595 and HUD Regulations.
  • Ensure program policies and procedures are in agreement with contractual requirements, guidelines, regulations and directives 
  • Improve agency wide program compliance with regulatory requirements.
  • Provide staff consultation and training in relevant city, state and federal regulatory requirements and policies.
  • Provide oversight in conducting agency wide annual Client Satisfaction Survey 
  • Organize and conduct agency-wide internal program mock audits. Develop and present outcome reports to programs, VP and CPO.
  • Oversee QI reporting activities including but not limited to incident data, charting compliance and qualitative outcomes, rent outcomes, client occupancy and vacancy outcomes. 
  • Develop and use charts, tables, diagrams presenting program data into understandable and readable visual summaries and reports.
  • Oversee employee AWARDS, CAPS, HASA web, CAIRS and DHS CARES training.
  • Coordinate Critical Incident reviews, complete NIMRS reports, and complete quarterly and annual incident trend reports. Conduct incident investigations and reviews including incident follow up and tracking/gathering supporting documentation
  • Provide reports as requested by Comunilife and/or funding agencies.
  • Assist the Chief Program Officer or designee with special projects, analyze, research, and prepare reports as required.

Qualifications

Education and Experience Requirement(s) 
A Master’s degree in social work, public health, health administration, or related field; Five (5) years of nonprofit administrative, program operation and coordination experience.  Experience and oversight with Program Performance Improvement Planning, Quality Insurance projects and Incident Management.  Bilingual in Spanish/English a plus.
