Job Summary

 Working in our Federally Qualified Health Center and under the direction of the Assistant Medical Director, and in collaboration with the Health & Wellness Center team, the License Practical Nurse (LPN) will focus on long-term acute care and preventative treatment of at-risk individuals. The LPN, a pivotal figure in our team, will perform clinical nursing functions based on the health center's needs. They will assist practitioners and perform other nursing procedures as assigned in the clinical areas and play a vital role in patient education, an essential aspect of our healthcare mission. The LPN will coordinate with the health care team to facilitate the highest quality of health services for patients. LPN will also provide telephone triage, arrange lab work, administer medications, and provide educational outreach. This position will be hired full-time within the Health & Wellness Center and will work with patients during all operating hours, including weekend hours.  This position requires in-person presence at CHNY’s Manhattan location; there is no opportunity for remote work.

 

DUTIES AND RESPONSIBILITIES

(Responsibilities to include but not limited to the following)

  • Administers medication and injections accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative response.
  • Follows-up on patient medical care emergency room follow-ups, including correspondence
  • Intakes patients and performs vital signs, venipuncture, and EKG as necessary.
  • The LPN will order medications, vaccines, and medical supplies as needed. This responsibility demonstrates their proactive approach to managing resources and ensuring the health center is well-stocked, which is crucial for the smooth operation of our health services.
  • Check emergency bag monthly and refrigerator temperatures daily.
  • The LPN will provide health counseling to patients, including HIV counseling. This is a vital aspect of our patient care and our commitment to public health. This role is integral to our mission and the well-being of our patients.

SALARY:  $56,000 - $58,000

Qualifications

Qualifications

  • Graduated from an accredited School of Nursing. Associate degree in nursing required
  • Current License to practice as a License Practical Nurse in NYS
  • CPR required at hire
  • Prefer 2-3 years experience in a Federally Qualified Health Center or Urgent Care with a strong preference for experience working with HIV testing, education, and treatment.
  • Proficiency in Electronic Health Record, eClinicalWorks preferred
  • Strong clinical skills
  • Strong technical skills, including phlebotomy, medication administration
  • Excellent Communication (both oral and written), interpersonal, and organizational skills
  • Excellent organizational and time management skills
  • Compassionate and understanding for people experiencing Homelessness
