Job Summary

The Rapid Re-housing Case Manager will provide services for up to 50 young adults aged 18-24 years residing in scattered site permanent supportive housing located throughout the Bronx, Brooklyn and Harlem. Reporting to the Program Coordinator of Rapid Rehousing and Director of Supportive Housing Programs, the Case Manager is an energetic, creative, and diligent member of a team that includes additional case managers, a Real Estate Coordinator and other supportive staff and services located in the Bronx. These services include mental health, education and employment, and health and wellness. 

The Rapid Re-Housing Program is a supportive housing program that is primarily funded by the US Department of Housing and Urban Development (HUD). Young adults live independently in private apartments that are subsidized for up to two years while working with a case manager to ensure that they are receiving services needed within their community. As the youth sign their own leases, they may remain in the apartment after the conclusion of services, which provides a stable exit plan from the program. 

The Case Manager will provide comprehensive case management services either in their office, in the youths’ apartments, or elsewhere in the community, to the youth who are on their caseload including: creation of an individual service plan; coordination and referrals of services, individual counseling, advocacy; monitoring of youths’ efforts and challenges in completing goals; and celebrating youths’ goal achievements. The case manager will work closely with the youth and Real Estate Coordinator to ensure youth are paying their portion of their rent on time and addressing arrears if needed. 

The Case Manager will contribute to a program culture that is person-centered, holistic, focused on positive youth development, and committed to continuous improvement. The Case Manager is expected to participate in agency wide training on youth development, the Housing First model, trauma-focused principles and de-escalation strategies for the benefit of providing appropriate services to youth residing in their independent apartments. Case Managers will also be expected to attend staff meetings including case review meetings to assess youths’ progress, as well as quarterly evening team meetings with all youth in the program. Additionally, this program will utilize evidence-based and other proven models; the Case Manager will participate in all training and implementation activities as needed. 

This position requires independent judgment and proactivity as the Case Manager will be working substantially in the field, making home visits regularly and otherwise assisting the youth to make community connections for long-term success. The Case Manager must also be comfortable in building and maintaining rapport with landlords and property managers. Flexibility, a willingness to be available by email or phone in off-hours, and excellent communication and problem-solving abilities are critical to successfully fulfilling the requirements of this position.

 

DUTIES AND RESPONSIBILITIES

 (Responsibilities to include but not limited to the following)

Case Management

  • Provides integrated counseling and case management services that effectively engage young adults and promote personal growth and wellness, housing stability, and ongoing accomplishment of service plan goals. 
  • Collaborates with youth to assess strengths and challenges and develops an individual service plan that reflects the youth’s progress and stated goals. 
  • Meets with youth regularly to determine progress with goals.
  • Advocates on behalf of youth with external stakeholders. 
  • Collaborates with and makes needed referrals to community providers. 
  • Ensures HUD mandates are followed in all cases and up-to-date in CHNY’s 
  • electronic case management system, Efforts to Outcomes (ETO). 
  • Creates and maintains confidential records, charts, and logs for youth, documenting all relevant information from individual and group interventions and with collateral services. 
  • Ensures that youths’ files are compliant with all regulatory and other requirements. 
  • De-escalates crises; attends to emergencies following agency protocol. 
  • Assists with property management issues in reporting to landlords and ensuring complete work orders. 

Team Participation 

  • Works as a collaborative team member, ensuring all youth have access to high-quality services and live in a safe setting. 
  • Participates in and facilitates regular case conferences. Ensures team members are fully apprised of information regarding youth as needed. 
  • Participates in staff team meetings and training, individual supervision meetings, and meetings with youth. 
  • Assists with audit preparation and dry and live audits of the program.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or a related field preferred and successful experience in a related field (at least one year) or a High School Diploma along with four (4)years of experience in case management, social services, youth development or related fields.
  • Demonstrated knowledge of and experience working directly with young people through approaches that emphasize resiliency, strength-based youth development, and trauma-informed care required. 
  • Strong multi-tasking skills with demonstrated ability to manage multiple projects at once required. 
  • Excellent verbal, written, and problem-solving abilities and an upbeat demeanor are required. 
  • A strong focus on child protection and ensuring clients' safety is required. 
  • Experience working with diverse groups is required. 
  • Overall flexibility in work schedule required, including weekends, evenings, and holidays if needed. 
  • Experience utilizing evidence-based and other social service models preferred. 
  • The ability to travel throughout New York City, regardless of weather, is required.
