POSITION SUMMARY: Our Certified Nurse Assistant is responsible for providing optimal patient care under the direction of the Nurse on Duty, while maintaining a safe environment
DUTIES & RESPONSIBILITIES:
Ability to follow direction and provide patient care as assigned by nurse.
Able to prioritize care for optional patient outcome.
Establishes good communication with other members of the health care team. Is consistent in reporting changes in patient condition or behavior to the charge RN or other staff RN or LPN.
Assists in room preparation for patient admissions, returning surgical patients, transfers, emergencies or discharges. Maintains a safe environment.
Transports patient to and from various departments as requested.
Provides post-mortem care and assists in transporting of the deceased.
Follows hospital policies and procedures and maintains a safe environment.
Utilizes the electronic medical record (EMR) for direction to care for the patient according to nursing and physician orders within abilities of certification.
Provides and assists in all aspects of personal care as:
Bed Bath/Tub Bath
Shampoo in tub or bed
Peri care and nail care and shaving
Oral Hygiene
Urinal, bedpan, commode, bathroom assistance
Meal preparation of patient and assistance with feeding.
Provides and assists in areas of patient integumentary system:
Promotes ambulation, assists patient
Transfers patient safely, use of gait belt, Hoyer lift
Range of motion exercises
Positioning of patients every 2 hours and as needed.
Skin care
Provides and assists in specialized, treatments and areas of patient care:
Isolation needs recognized and addressed
Soap suds, oil retention or fleets enema
Colostomy care
Vaginal douche
Glucose testing with glucometer
Ng tube equipment monitoring
Understands protocol for:
Vital signs T.P.R and BP – manual and automatic
Post Op Vital sign routine
Height and Weight
Recording intake and output (I&O)
Collection, labeling and transporting of specimens.
Assists patients with handling and care of clothing and other personal property including:
dentures, glasses, contact lenses, hearing aids, prosthetic devices.
Assists in maintaining a safe and clean environment by:
Organizing patient rooms
Maintaining, cleaning, and stocking storage room and nurse servers and makes requisitions for items needed.
Cleaning and preparing equipment for disinfection
Reporting equipment that is out of order
Appropriate handling of supplies and clean and dirty linen.
Keeping staff RN or Charge RN up to date on any problems.
Makes frequent rounds on patients. Participates in hourly rounding. Collects rounding checklists and forwards to Manager. Relays complaints or requests appropriately.
Identifies specific needs of all patient groups and can assist in their care. Seeks direction when needed.
Able to cover desk when needed, after completion of orientation. Refer to Ward Clerk job description.
At no time administer medications to patients.
At no time changes IV equipment. Discontinues saline locks if specially trained.
Documents all care given appropriately and in a timely manner in the EMR system.
Documents in “Real Time” at all times possible.
Establishes and maintains interpersonal relationships with patients, families, visitors and other hospital personnel. Assures patient confidentiality is maintained and always acts as a patient advocate.
Attends in-service education programs to learn new treatments, procedures and developmental skills.
Attends all mandatory meetings.
Admit patients in the absence of the admitting clerk
Attends all staff meetings when able. Keeps updated by reading minutes in a timely manner when unable to attend.
Has an understanding of Quality Improvement program and needs. Reviews bulletin board routinely. Assists in accumulating data for Quality Improvement when asked.
Reviews Pillar Board to keep current departmental and organizational information.
Knows role in the event of fire, disaster or code. Applies same in a drill situation.
Knows procedure for calling/paging of on-call personnel.
Recognizes importance and follows Chain of Command.
Works with Nurse Clinical Assistant to keep EMR skill up-to-date, knowledge of version enhancements and in-servicing. Assists with temporary registration.
Participates in mandatory hospital wide staff education requirements, example: Care Learning, Infection Control requirements, etc.
Contributes to a positive work environment and to the overall team effort of the nursing unit. Adheres to the Behavioral Standards policy.
Assists other staff in patient care as needed.
Demonstrates appropriateness in meeting objectives in age-specifics.
Performs other duties as assigned.
