Description

Job Purpose: This is a professional position that is responsible for the care and treatment of adults with mental illness. The intake coordinator will complete all initial assessments and documentation, develop, and implement the initial treatment plan, and will provide coordination of care as needed.

1. Conduct the intake of new clients to the program. This will include the tracking of the intake process and completing initial paperwork for each new client in a timely manner and accordance with Milwaukee County and State regulations and guidelines.
2. Monitor and ensure that intake documentation meets all relevant local, state and federal standards, including DHS 36 and Title 19 regulations.
3. Maintain a caseload of clients based on program needs and assist with teaming as needed.
4. Maintain system of medical records management.
5. Responsible for accurate documentation and maintaining case file as required by agency policy, government regulations and applicable local, state and federal codes.
6. Responsible for meeting department designated monthly billing quotas, EHR note entry within 72 hours and biannual treatment plans as assigned.
7. Attends and participates in staff meetings, in-service training seminars and conferences as required.
8. Responsible for adhering to agency policies and procedures while carrying out daily responsibilities.
9. Providing client coverage for other staff members as assigned.
10. Attendance at community-based meetings as assigned including, but not limited to, MC3 Change Agent meetings.
11. Access to a personal vehicle, a valid Wisconsin driver’s license and insurance in conformity with agency limits

Qualifications

Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Bachelors in the field of Social Work, Counseling, or related Human Services Field. Master's Degree is preferred

Travel outside of office: 60%

Knowledge – Skills – Abilities:

  • Preferred Certification as a functional screener is desired but will provide training as needed.
  • Ability to clearly convey complex instructions and ideas both in writing and verbally.
  • Knowledge to use work related equipment and applications such as cell phone and texting, computers, photo copier, Microsoft Office, and Teams.
  • Knowledge with individuals with dual diagnoses is preferred.
  • Knowledge with motivational interviewing is preferred.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements: Exerting 10-20 pounds of force occasionally. Reaching, extending hand(s) and arm(s) in any direction. Standing for sustained periods of time. The occasional use of stairs. Walking, moving about on foot to accomplish tasks, pushing, pulling. Lifting, raising objects from a lower to a higher position or moving objects horizontally from position to-position. Fingering, picking, pinching, grasping items, typing, or otherwise working.

Talking, expressing, or exchanging ideas by means of the spoken and written word. Hearing, perceiving the nature of sounds at normal speaking levels with or without correction

Visual Acuity: The worker is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned, preparing, and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading with or without adaptive equipment

Working Conditions: Work is performed largely in the community but may at times include an office environment or other various settings. The worker is subject to inside and outside environmental conditions, loud noises, and fluctuating temperatures
