SUMMARY

 

The primary purpose of your job position is to each of your residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisor.

 

DELAGATION OF AUTHORITY:

 

As a Certified Nursing Assistant, you are delegated the authority, responsibility, and accountability necessary to carry out your assigned duties.

 

ESSENTIAL DUTIES & RESPONSIBILITIES include the following.  Other duties may be assigned.  Every effort have been made to identify the essential functions of this position.  However, it in not states or implies that these are the only duties you will be required to perform.  The omission of specific statements of duties does not exclude them from the position if the work is similar, or is an essential function of the position.

 

Use the wristband to identify the residents before administrating treatments, serving meals, etc. as necessary.

 

Record medical and administrative information in established charting and documentation policies.

 

Medical records are maintained in an informative and descriptive manner.

 

Maintain the Daily Census Report and submit to Nurse Supervisor/Charge Nurse.

 

Maintain an up-to-date roster of residents for your assigned unit.

 

Answer telephone, page calls; deliver messages to residents, etc., as necessary.

 

Record appropriate resident identification data records, wristbands, ID cards, etc., as required.

 

Transcribe physician orders, medical plans, etc., as required.

 

Report to Nurse Supervisor/Charge Nurse discrepancies found in transcribing physicians’ orders, diet orders, changes, charting, etc.

 

Review MARS and AARS to completeness of information transcription of the physician’s order, legibility etc., as directed.

 

Record vital signs as directed.

 

Forward new diet orders and/or diet changes to the Director of Food Services.

 

Keep nursing service personnel informed of resident’s appointments.

 

Assist in arranging for and making resident appointments for diagnostic and therapeutic services.

 

Admit, transfer and discharge residents. Assist in arranging for transportation, packing residents’ belongings, escorting them to discharge/transfer area, loading, etc., as necessary.

Complete necessary medical and administrative records upon the resident’s admission, transfer, and/or discharge.

 

Notify the Nurse Supervisor/Charge Nurse should a resident be discharged without a physicians’ order.

 

Forward complete charts of discharged residents to the Medical Records Department.

 

Deliver mail, flowers, etc., to residents as necessary.

 

Direct visitors to resident rooms, office areas, etc., as necessary.

 

COMMITTEE FUNCTIONS

 

Assist in preparing written and/or oral reports of the nursing service programs and activities to submit to such committees as directed.

 

Provide data to the Quality Assurance and Assessment Committee as requested.

 

Participate in developing the agenda for the Care Plan Committee meetings as necessary.

 

Implement recommendations from established committees as instructed.

 

PERSONNEL FUNCTIONS

 

Assist in the orientation of newly assigned personnel as directed.

 

Interpret department policies and procedures to personnel, resident visitors, etc., as directed.

 

Assist the Nurse Supervisor/Charge Nurse in developing work assignments, schedules, etc., as required.

 

Develop and maintain a good working rapport with interdepartmental personnel, as well as other departments within the facility.

 

Create and maintain an atmosphere of warmth, personnel interest and positive emphasis, as well as a calm environment throughout the unit shift.

 

NURSING CARE FUNCTIONS

 

Inform nursing services personnel of new admissions, their expected time of arrival, room assignment, etc.

 

Greet newly admitted residents upon admission. Escort to room as necessary.

 

Assist in escorting residents to/from activity and social functions.

Encourage attending physicians to record and sign progress notes, physicians’ orders, etc., on a timely basis and in accordance with current regulations.

 

Assist in record keeping for nurse aide trainees.

 

Report administrative problem areas to the Nurse Manager and assist in developing and implementing corrective action.

 

Call funeral homes requested by the family as directed.

 

STAFF DEVELOPMENT

 

Assist as necessary in the orientation of newly assigned personnel to your unit.

 

Attend in-service training programs as scheduled.

 

Attends and participates in annual OSHA and CDC in-service training programs for Hazardous Communication, TB Management, and Bloodborne Pathogens Standards.

 

Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession.

 

SAFETY AND SANITATION

 

Follow established safety regulations in the use of equipment and supplies.

 

Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids.

 

Assist in maintaining nursing services work areas (i.e. nurses’ station, medicine preparation rooms, etc.) in a clean and sanitary manner.

 

Participate in all fire safety and disaster preparedness drills in a safe and professional manner

 

Participate in the implementation, and maintenance of the infection control program for monitoring communicable and/or infectious diseases including TB, among the residents and personnel.

 

Follow established infection control procedures when isolation precautions become necessary.

 

Follow established procedures in the use and disposal of personal protective equipment.

 

Report hazardous conditions or equipment to the Nurse Supervisor/Charge Nurse.

 

Follow established hand washing techniques.

 

EQUIPMENT AND SUPPLY FUNCTIONS

 

Inform the Nurse Supervisor/Charge Nurse of your equipment and supply needs.

 

Follow established preventive maintenance procedures for maintaining your equipment and supplies in a safe and operable manner.

 

CARE PLAN AND ASSESSMENT FUNCTIONS

 

Assist as necessary in the development of a written assessment of the nursing needs of each resident.

 

Assist as necessary in the development of a written plan of care for each resident.

 

Ensure all personnel involved in providing care to the residents are aware of the resident’s care plan and nursing personnel refer the resident’s care plan prior to administering daily care to the resident.

 

RESIDENT RIGHTS

 

Maintain confidentiality of all resident care information to assure resident rights are protected.

 

Treat all residents fairly, and with kindness, dignity and respect.

 

Knock before entering a resident’s room.

 

Review complaints and grievances made by the resident and make a written/oral report to the Nurse Supervisor/Charge Nurse. Follow the facility’s established procedures.

 

Maintain a written record of the resident’s complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.

 

Report all allegations of resident abuse and/or misappropriate of resident property.

EDUCATION and/or EXPERIENCE

 

High School Diploma or GED

 

Previous experience in medical field.
