Mental Health Housing Case Manager

Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Mental Health Housing Case Manager. The Mental Health Housing Case Manager works in Cascade AIDS Project’s Housing and Support Services Department to provide services that include housing case management, goal planning, information and referral services, advocacy with and on behalf of clients, and eviction prevention. The person in this position will work with up to 30 individuals in our supportive housing program who are living with HIV and have co-occurring mental health and addiction issues and are homeless or at risk of becoming homeless

The Mental Health Housing Case Manager will support CAP’s efforts to increase staff knowledge of mental health issues as it related to housing and homelessness and will provide suggestions and support to department staff who are also working with PLWHA People Living with HIV/AIDS) who have a co-occurring mental health issue.

  

 

This is a Full-Time role working at our Davis st location in the Old Town area of Portland, OR. The schedule for this role is a Hybrid Monday-Friday 8:30am-5:00pm

Starting pay is $58,189.59 a year. The Mental Health Housing Case Manager’s pay increases with tenure and the top of the pay scale is $65,986.59. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

 

 

 

Who You Are

  • Master’s Degree in field of Mental Health (Social Work, Counseling, or Psychology)  
  • Prior experience working with persons with severe and persistent mental health issues  
  • Prior experience providing case management (or similar) services  
  • Prior experience working with people experiencing homelessness 
  • Successful experience working with ethnic, racial, economic and sexually diverse populations and persons who have experienced homelessness, persons with a mental illness and/or substance addiction  
  • Ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical discussions for how CAP can better serve our communities and mission  
  • Demonstrated ability to effectively collaborate with community stakeholders
  • Able to travel throughout the service area on a regular basis     

 

 

What You’ll Do

  • Provide housing placement, supportive case management, and eviction prevention with and on behalf of clients who are homeless or at risk of becoming homeless and have co-occurring mental health issues using a supportive, strengths-based model that promotes client self-determination and independence  
  • Work in collaboration with CAP staff, medical case managers, medical providers, or other providers involved in client care, to provide coordinated comprehensive care to PLWHA with a focus on housing stability  
  • Assist in the development and implementation of equity and inclusion initiatives within the department  
  • Support the implementation of CAP’s organizational equity initiatives  
  • Participates and engages in equity-based trainings, workshops, and team discussions  
  • Compile resource information about mental health services and share with colleagues   
  • Carry an active client panel of approximately 30 program eligible clients, assisting with housing placement, eviction prevention, crisis resolution, housing planning, advocacy, mediation, and information and referral
