Cascade AIDS Project is excited to announce that we are hiring skilled and team-oriented Carelink Navigator. The CareLink Navigator assists participants with navigating the healthcare, mental healthcare, and substance use treatment systems as well as provides support in the form of referrals to housing and shelter resources as this is a critical component to their stability and medical engagement.

 

 

This is a Full-Time role working at our Davis st. location in NW Portland. The schedule for this role is a hybrid, Monday-Friday. 8:30am – 5:00pm.

Starting pay is $22.77 an hour which is equivalent to a Salary of $47,370.59. The Carelink Navigator’s pay increases with tenure and the top of the pay scale is $26.52. This is a union represented position, so the compensation, benefits and conditions of work are collectively bargained.

 

 

Who You Are

  • Bachelor’s Degree in human/social services field (social work, public or community health, psychology) or related field or equivalent experience.
  • A minimum of one-year experience providing case management services to people who have experienced houselessness, mental health and/or substance use needs
  • Experience working with ethnic, racial, economic, and sexually diverse populations and a strong understanding of issues relevant to people living with HIV, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery
  • Demonstrated computer and keyboard proficiency using Microsoft Office software (Word, Excel, Outlook) and working knowledge of the internet
  • Excellent written and oral communication skills
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to: engaging in training workshops, team and department meetings, and critical discussions for how CAP can better support the communities that we serve. 

 

 

What You’ll Do

  • Develop and maintain professional relationships with agencies that serve people living with HIV/AIDS (PLWHA)
  • Carry a caseload of approximately 10-15 clients
  • Establish rapport with clients using a supportive strengths-based model that promotes client self-determination and independence.
  • Assess clients’ involvement in HIV services, identify barriers to care and readiness to access care, including client knowledge of HIV status, coping resources, social support, chemical dependence and mental health issues
  • Establish rapport with clients and work with them to develop and carry out individualized goal plans to facilitate engagement and retention in medical care, substance use treatment and mental health services
  • Make appropriate referrals to CAP programs, mental health, substance use, HIV prevention, or other community partners as needed to support client self sufficiency and provide support to clients in accessing these services
