GENERAL PURPOSE: PCMH Coordinator
Under the direction of the PCMH Manager, the PCMH Coordinator will promote and support the PCMH model of care. The role involves maintaining expertise in all PCMH competencies, developing training materials, and training staff accordingly. The PCMH Coordinator will audit data, create reports, and lead initiatives aligned with the PCMH model of care. Performance will be monitored, and quality control plans implemented to identify improvements. Data collection will be turned into information; information will be turned into insight; insight into business decisions made by Ampla Health.
MAIN RESPONSIBILITIES AND DUTIES: PCMH Coordinator
- Promote the PCMH model of care within the organization.
- Maintain and exhibit expertise on all PCMH competencies.
- Travel to Clinical sites for training and meetings.
- Develop and deliver training materials on PCMH competencies to staff.
- Audit data and generate PCMH reports.
- Participate in and lead initiatives that align with the PCMH model of care.
- Provide training in PCMH to onboarding staff.
- Train Nursing Supervisor and Site Administrators in PCMH reporting and PCMH competencies.
- Utilize databases, collect data, and will perform data analytics and other strategies to optimize statistical efficiency and quality.
- Acquires data from primary or secondary data sources for grant reports.
- Identify, analyze, and interpret trends or patterns on data sets.
- Works with IT to identify data by reviewing computer reports, printouts, and performance Indicators to correct EMR as it relates to PCMH issues.
- Locates and defines new processes to improve PCMH Opportunities
- Works with Insurance companies for reporting issues as needed.
- Adds new immunizations to the EHR system.
- Builds templates with IT.
- Works with the CCSO to collect data for the drug representatives.
- Works on the Care Planning and TCM modules for PCMH.
- Runs reports for HRSA for monthly COVID-19 Submissions.
- Trains the 340B and Surgicare Program and follows with reports.
- Conducts 340B Quarterly Audits and works with Dr. Nota on annual internal audits.
- Ensure compliance with all Ampla Health standards, procedures, and protocols.
- Commitment to achieving annual PCMH recognition.
- Perform other duties as assigned by the supervisor.
- Will assist with ordering of HPP and other supplies.
- Will Assist with the Relias Staff education system.

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES: PCMH Coordinator
- Medical Assistant certificate required.
- High school diploma/GED required.
- Proficiency in Microsoft Word, Excel, EMR, and PowerPoint.
- Valid California driver’s license with a safe driving record.
- 3-5 years of experience in a clinical setting.
- Strong organizational, multitasking, problem-solving, and time-management skills.
- Ability to work independently and manage multiple projects.
- Ability to maintain strict confidentiality (HIPAA) with sensitive medical information and foster an ethical work environment.
- IT experience preferred.
- Experience with PCMH Accreditation preferred.
- Strong teamwork and collaboration skills.
COMMUNICATION SKILLS: PCMH Coordinator
- Neat and legible handwriting.
- Effective communication with the public and patients.
- Proven ability to communicate effectively, both verbally and in writing, with all levels of staff.

WORKING CONDITIONS & PHYSICAL REQUIREMENTS: PCMH Coordinator
- Must be in good health and able to lift up to 40 lbs.
- Operate standard office equipment such as a computer, telephone, fax machine, copier, etc.
- Ability to reach above shoulder level, bend, squat, sit, stand, stoop, crouch, kneel, and twist/turn.
- Ability to work independently in a fast-paced environment with frequent interruptions.
