DESCRIPTION

The Case Manager is a part of the multidisciplinary Addiction Care team and has a responsibility to maintain safety for themselves and their charges, guiding and directing clients through the discharge process via collaboration with the treatment team, clients, families, and outside referral sources. The Case Manager's responsibility is to assist doctors and nurses with treating patients who have various behavioral problems, such as substance abuse, post-traumatic stress disorder, mental instability, and physical and emotional abuse. This may be as simple as keeping patient files confidential and secure from unauthorized access or assist in redirecting aggressive patients. In many cases, the ability to emotionally support patients, remain calm and professional, and use good interpersonal skills can defuse a volatile situation.   
 
Case Manages evaluate client needs, discuss their condition with clinicians, plan their homecoming or transfer, and arrange any follow-up appointments, e.g., doctor visits or individual therapy sessions. Case Manager/Discharge coordinators are skilled client advocates who play a critical role in helping current patients develop personalized continuing care plans by organizing an effective post-treatment strategy for clients to heal fully either at home or within another care facility. Careful assessment of each client's individual needs and ensuring a smooth and safe transition through the recovery process is required. It's the overall goal to empower discharged clients as they embark on recovery.   

DUTIES AND RESPONSIBILITIES

  • Includes, but is not limited to, delivering as much of the "help" or service as possible, making referrals to multiple outside formal services (E.g., Psychiatric appointments, Primary care medical appointments, and Medication-Assisted Treatment (MAT) appointments)  

  • Maintains and coordinates communication with outside case managers and probation officers (Updating them on the progress the patient is making in treatment)  

  • Assists the patient in developing a safe post-discharge transition plan (E.g., setting up continued care services such as IOP as well as assisting in finding sober living facilities)  

  • Protects client health and well-being by devising personalized recovery plans after treatment  

  • Handles the processing of discharge documentation and forwarding them to appropriate clinicians and programs as needed  

  • Audits EMR documentation to ensure compliance with institutional guidelines  

  • Maintains data integrity for electronic medical records  

  • Verifies medical insurance information for each client to transition and schedule continuing care within their payor constraints  

  • Meets one-on-one with clients to instruct them on proper continuing care and answer any questions  

  • Manages assigned case of clients  

  • Provides referrals for clients to counseling, psychiatric, or medical care as needed for improved long-term health outcomes  

  • Complies with the regulations and policies of the facility  

  • Obtains all pertinent ROI’s for all referrals   

  • Schedules discharging client transportation as needed  

  • Communicates necessary discharge information with the referral source(s) and Buena Vista treatment team  

  • Engages in outreach and encouragement with both patients and their loved ones, educating on the benefits of substance abuse treatment in a supportive and compassionate tone  

  • Maintains consistent communication with all parties regarding pertinent discharge information via the designated organizational platforms  

  • Keeps clients safe by following safety policies, procedures, and regulations  

  • Maintains confidentiality and complies with HIPAA, PHI and PII laws and regs  

  • Maintains a schedule that meets the needs of the organization 

ADDITIONAL NOTES

  • Associate Degree in a human service field preferred 

  • Have a working knowledge of case management, setting appointments, outside referrals medical transport preferred 


PHYSICAL REQUIREMENTS

  • Work requires the following motions: bending, twisting, squatting and reaching 

  • You must have the ability to use tools and equipment required for the position 

  • Mental Focus: Ability to read and communicate effectively in English, both verbally and in writing 

Qualifications

MINIMUM QUALIFICATIONS AND SKILLS REQUIRED

  • 2+ years of experience working in the behavioral health field  

  • 1+ years of experience performing case management duties   

  • Must be 21 years of age or older  

  • Valid CPR, First Aid, and CPI certifications or the ability to obtain immediately upon hire  

  • Level one fingerprint clearance card  

  • Demonstrated proficiency with an electronic medical record system (EMR) and Microsoft Office products including Word, Excel, and Outlook  

  • Working knowledge of HIPAA PHI security requirements  

  • Strong affinity for customer service  

  • Excellent organizational and time-management skills  

  • Excellent oral and written communication  

  • Strong interpersonal communication skills and the ability to maintain a positive collaborative relationship among all constituencies  

  • Ability to work in a fast-paced, challenging and dynamic environment  

  • Excellent organizational, time-management, and problem-solving skills

EDUCATIONAL REQUIREMENTS
High School Diploma/GED
