In an environment of continuous quality improvement, the Staff Pharmacist is responsible for complying with all applicable hospital policies, procedures, codes and standards. In the absence of the pharmacy director, may be required to perform the essential duties of the pharmacy director. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times.
Job Functions:
Pharmacy Operations:
1. Provides coverage of central inpatient pharmacy and decentralized areas where pharmacy provides services as requested. Participates in inventory and stock control functions. Actively oversees and supervises pharmacy technicians and provides direction and training as necessary. Provides drug information to the hospital’s professional staff and patients as necessary. Assists with preparation of sterile products and chemotherapy/compounding. Oversees or assists with Pyxis restocking and troubleshooting as needed.
Order Entry:
2. Interprets, clarifies, and supervises the filling of physician orders using current pharmacy practice laws, acceptable pharmacy practices, available resources, and clinical judgment. Performs prospective drug utilization evaluation with every order entry.
Clinical activities:
3. Participates in clinical activities such as medication profile review, drug therapy/pharmacokinetic consults and dosing, renal dosing adjustments, parenteral nutrition dosing, inpatient multidisciplinary rounds and patient education as needed. Monitors drug therapy and pertinent lab information/drug levels and provides appropriate interventions when required. Assists with antibiotic selection compared to culture and sensitivity. Monitors and reviews laboratory values for patients receiving parenteral nutrition. Assists and recommends IV to PO conversions when necessary. Responds to code blue situations and assists with medication interventions.
Quality Assurance:
4. Assists in the coordination and implementation of quality assurance activities, and assists with reporting and prevention of medication errors and adverse drug events.
5. Assumes all other duties and responsibilities as requested by the director of pharmacy.
