This position is region wide across the 6 counties of Erath, Hood, Johnson, Palo Pinto, Parker, and Somervell. Assignment will vary from day to day based upon where the need is that day. Salary is dependent upon education and experience.
Job Description
The Crisis Case Manager (CCM) provides moderately complex case management work. The CCM will provide services and interventions to a variety of ages, including children, youth, and adults. This position is primarily clinic-based with some community interventions as needed to meet the needs of those served. CCM performs clinical screenings, assessments, and service eligibility evaluations based on consumer needs. CCM will coordinate and link to appropriate services and monitors consumer progress with meeting recovery goals. Delivers interventions such as crisis resolution, case management services, and discharge planning. Provides clinical services to address the need for ongoing services and/or inpatient admission. Provides crisis follow-up and relapse prevention services when appropriate. This position requires travel, flexible schedule, and participation in the on-call rotation. A minimum of three days per month of participation in the on-call rotation will be required. Must be familiar with and/or willing to be trained in trauma-informed care. This includes understanding, recognizing, and responding to the effects of all types of trauma.
Duties and Essential Job Functions
Crisis Case Manager will interact in a timely manner with supervisor and staff. Extensive use of an electronic health record and electronic communication will be required. Duties include but are not limited to the following:
Accurately complete a uniform assessment to identify an individual’s needs and strengths as appropriate
- Must be able to utilize an electronic health record system to document collaboratively and concurrently with consumers across all processes, including assessments and service planning
- Communicate with referring entities, internal and external providers
- Maintain a flexible work schedule while demonstrating effective time management to meet needs of individuals served
- Travel between locations as assigned
- Participate in data collection activities and peer review processes as requested
- Other duties as assigned by supervisor
