VP of Care Management

We are seeking a dynamic and experienced Vice President of Care Management to lead our care management team and operations at ilumed. Responsibilities include shaping, driving, and executing a comprehensive care management strategy that enhances patient outcomes, supports provider performance, and drives shared savings. This individual will oversee all aspects of care management, including coordination and utilization oversight, with direct accountability for the performance and professional development of a multidisciplinary care management team.

This role requires a visionary leader and skilled operator who can translate high-level strategic objectives into scalable, measurable programs. The VP will lead with a commitment to evidence-based care, team empowerment, and a relentless focus on achieving clinical and operational Key Performance Indicators (KPIs) to enhance patient outcomes and financial success. The successful candidate will bring a strong command of value-based care principles, deep experience in healthcare delivery innovation, and a proactive approach to solving complex system challenges.

What You'll Do:

• Lead the development and execution of ilumed’s care management strategy.

• Oversee daily operations of the care management team and ensure delivery on KPIs.

• Build and mentor a high-performing clinical team focused on innovation, accountability, and quality care.

• Partner cross-functionally with clinical and operational teams to optimize workflows and performance.

• Use data-driven insights to enhance decision-making and drive continuous improvement.

• Represent ilumed in stakeholder engagements and help foster strong external partnerships.

What We’re Looking For:

• 10+ years of progressive leadership in care management, with deep experience in value-based care.

• A Bachelor’s or Master’s degree in Nursing, Healthcare Administration, or a related field.

• Active RN licensure in good standing.

• Proven success in managing clinical teams, aligning with KPIs, and scaling care operations.

• Strong background in care coordination, utilization oversight, and strategic planning.

• Excellent communication, critical thinking, and project management skills.
