Job Summary:
Oversees the daily operations of the ACT program, to include but not limited to referral management, scheduling work assignments, training new employees, monitoring timecards, supervising staff performance, and assisting the leadership of Care Coordination with recommendations in the hiring and termination of staff members. Conducts weekly staff supervision with focus on providing strategies to support clients with care coordination needs. Identifies administrative and personnel deficiencies; collaborates with leadership as appropriate. Serves in a dual capacity as a limited direct service provider; meets and maintains assigned client caseload by rendering nursing services.
Provides services in the community with the framework of core ACT operating principles, professional nursing ethics, and state and city regulatory guidelines; such services include but are not limited to: inpatient, outpatient, and community-based visits. Delivers quality care through effective nursing assessment, triage and appropriate treatment planning. Collaborates with the psychiatry provider in supporting and coordinating client medical needs, to include but not limited to: medications (injections and oral), primary health care, dental, and vision services. Manages psychiatric crises and collaborates with the team to assure the best outcome for client’s safety. Actively engages with community partners such as jail/probation, emergency rooms, assisted living communities, and other partners involved in a client’s plan of care.
