The Medical Coordinator provides personalized care and delivery medical services that uphold program policies and procedures; in accordance with licensure, client's rights, medical concerns, and age-appropriate environment is in place. They are responsible for coordination and delivery of medical services to unaccompanied children, ensuring clients receive initial medical examination, immunizations, medical screenings, and dental evaluation within program deadlines; in addition, must update the client's file records and the UC portal corresponding medical assessments, follow up appointments, and all medical services. Medical coordinators have to work closely with the registered nurse to ensure the wellbeing of all children in care, with medical providers, and other member of the team. They routinely assist with the daily medical activities, transportation, medication log, monitoring Medication Administration Records (MAR / Prescription and Over the Counter Medication), reporting, and writing SIRs according with the current policies, requesting TARs, and others related with medical. Medical Coordinators participate in quality activities through their active contribution in Treatment Plan Meetings, departmental meetings, program meetings, and required ongoing trainings. This position is carried out in designated shelter locations, school, and group home settings.

What you Deliver

  • Ensure the safety and security of all children in care at all times.
  • Provide quality care and supervision to clients in accordance with ORR and TDFPS rules, program philosophy and procedure and client rights.
  • Maintain ratio and close supervision through all activities.
  • Must report immediately to the supervisor any doubts, questions, medical concerns, or procedure regarding client's health.
  • File medical records: make sure the information and medical files for each client are safe and kept confidential.
  • Effectively communicate information pertaining to the health clients according to the program policies and procedures to ensure quality services and client safety.
  • Adjust work to the plans and programs that the Registered Nurse implements.
  • Maintain accurate and update documentation regarding biographical data, admission assessment, medical progress notes, Medication List, Medication Log, MARs, TARs, Sickness/Allergy Forms, Medical instructions/recommendations, medical summaries, Safety plans, SIRs, IRs, and other as needed.
  • Keep ongoing training to compliance with the annual 40 hours training in accordance with state regulations, Office of Refugee Resettlement, and both Catholic Charities and St. Michael's Home for Children requirements.
  • Keep ongoing and mandatory trainings to maintain full agency, funder and licensure compliance. Besides, take online trainings as needed to renew the CAN Certification.
  • Assist in the-development, oversight, or provision medical services for clients in care as approved by your supervisor for safety and licensure compliance.
  • Look for assistance or advice from your immediately supervisor, if you have doubts or questions before performing an assigned task.
  • Perform other duties as assigned train such as participating in staff retreats, United Way tours, agency and community fairs and speaking engagements, assisting with special organizational projects/events, in order to promote agency programs.

Qualifications

The Expertise We're Looking For

  • High School Diploma or G.E.D.
  • Valid CNA (certification as Nursing Assistant)
  • One (1) year of experience in the child welfare field working with children and/or adolescents in a social service setting
  • Bilingual in Spanish in English.
  • Valid Driver’s license

  • Proficient typing/keyboarding skills and computer skills including but not limited to Microsoft Office applications such as Outlook, Word, Excel, and power point.
  • Excellent communication (verbal and written) and interpersonal skills.
  • Ability to work and function well in a diverse and multidisciplinary environment.
  • Capacity to and adjust/adapt to flexible schedules, prioritize activities, responsibilities, and work within fast- paced environment.
  • Knowledge of essential medical terminology and understanding basic health attention like, vital sign measuring, blood sugar monitoring, and Weight/height measure; as well, performance hearing and vision screening.
  • Ability to adapt to changing work requirements and varying volumes of work under pressure.
  • Knowledgeable of HIPAA, state and federal regulations governing confidentiality and release of information.
  • Support and train team members under Registered Nurse supervision and guidance.
  • Demonstrate knowledge and understanding about boundaries with kids, staff and volunteers
