What you Deliver

You work with supervisor to build and maintain successful case management team including regular contact with all enrolled clients, and tracking progress of clients toward completion of goals on service plan.

You provide guidance, mentorship, and support to Case Managers to enhance their professional development. Assist in the onboarding process for new Case Managers and ensure they receive comprehensive training. Serve as a source of knowledge for other staff members regarding case management and grant requirements.

You assess client eligibility for program services, complete intakes, and create individualizing service plans. Provide services identified on service plan in a timely manner. Make necessary referrals within the agency and outside of the agency, as needed. Ensure that services are provided in accordance with LEP policy.

You maintain client files, updates case notes, and ensures that all client information and activities are tracked in Client Track as well as Refugee Management Solutions.

You represent Catholic Charities at events with other local resettlement agencies, churches, and community organizations.

You document all contacts and services in client case files and maintains case note logs. Completes all required reports related to case management.

You perform other duties as assigned, such as assisting with outreach, training volunteers and interns, assisting with airport pick-ups, assisting with projects, etc.
