POSITION SUMMARY:   

As our Housing Case Manager (CRM), you will provide support services to participants in our Quest community that are part of our Culturally Specific houses, for those engaged in our Outpatient SUD Program in Multnomah County. The person in this position is committed, flexible, organized and solution oriented. They use these skills to identify and disseminate resources, foster community relationships, facilitate the housing application process and help clients secure safe and affordable housing.   

ESSENTIAL FUNCTIONS:    

  • Assist participants in meeting their housing goals 

  • Apply person-centered principles to build relationships, trust, and rapport with clients 

  • Assess client needs with housing and develop goal/action plans accordingly 

  • Utilize cultural competency, active listening, and motivational interviewing skills to assist clients in reaching their treatment goals 

  • Establish and maintain relationships with local agencies for an efficient referral process 

  • Collaborate with local agencies to coordinate services 

  • Travel with clients to and from recovery building and housing focused opportunities 

  • Work closely with internal and local Peer Support Specialists and Certified Recovery Mentors to maximize the quality of community service.  

  • Complete chart notes for client encounters and submit all program paperwork in a professional, timely manner 

  • Working closely with an integrated team 

  • Maintain compliance with applicable laws, rules, policies, and guidelines governing work &/or licensure/certification 

  • Attend regular clinical supervision meetings 

  • Perform other duties as assigned/needed 

Qualifications

KNOWLEDGE, SKILLS AND ABILITIES: 

  • Understanding of Fair Housing laws, and responsible for staying up to date on evolving laws 

  • Ability to manage time and meet deadlines for multiple projects at a time 

  • Basic computer and keyboard proficiency using Microsoft Office, Office 365 software, and our EMR 

  • Ability to be culturally conscious and work with a diverse caseload

  • Ability to work collaboratively as a team member as well as independently 

  • Ability to maintain accurate records and necessary paperwork in our EMR and other 

  • Must adhere to agency's non-discrimination policies 

MINIMUM QUALIFICATIONS: 

  • Meets the definition of a Peer as defined by the State of Oregon Administrative Rule: a) self-identified person currently or formerly receiving mental health services; OR b) a self-identified person in recovery from a substance abuse disorder, who meet the requirements for recovering staff in alcohol and other drug treatment programs; OR c) A family member of an individual who is a current or former recipient of addictions or mental health services.  

  • Must hold a Peer Support Specialist (PSS) and/or Certified Recovery Mentor (CRM) certification from a state-approved training program or be willing to become certified immediately. Prior experience, developing client goal and action plans 

  • Knowledgeable about the lived experience of addiction, recovery, MAR, and being involved in the corrections system.  

  • Applicants in recovery from substance abuse must have a minimum of 1 year clean & sober.

PREFERRED QUALIFICATIONS: 

  • Exceptional applicants will have experience facilitating support groups 

  • Preferred experience working within the recovery, HIV services, LGBTQIA2S+, BiPOC, and/or other communities we serve
