MD Home Health/MD Home Assist is one of the largest home health agencies in Arizona, providing medical and non-medical care to the greater metropolitan Phoenix area. Family-owned and founded 39 years ago in Phoenix, we are Medicare Certified and State Licensed, serving patients daily with a dedicated team of nurses, therapists, and caregivers.

We are seeking a dedicated Physical Therapist Assistant to join our team in the West Valley area. The ideal candidate will assist in providing physical therapy treatments and interventions under the supervision of a licensed Physical Therapist. This role offers an opportunity to work in patients home.

Responsibilities
- Assist in implementing care plans as per the Physical Therapist's instructions
- Provide support in conducting patient evaluations and documenting progress
- Aid in discharge planning and patient education
- Collaborate with the healthcare team to ensure comprehensive patient care
- Perform therapeutic exercises and activities with patients
- Maintain accurate patient records and reports

Work Location: On the road

Job Types: Full-time, Part-time, PRN, Per diem

Qualifications

Qualifications
- Degree in Physical Therapy Assistant program or equivalent
- Valid certification or licensure as a Physical Therapist Assistant
- Strong knowledge of anatomy, physiology, and physical therapy principles
- Familiarity with occupational health practices is a plus
- Excellent communication and interpersonal skills to interact effectively with patients and healthcare professionals
