Client Care Coordinator will serve clients across a variety of settings. Focuses on actively listening to clients and their caregivers, friends, and families. Client Care Coordinator will serve as an effective problem solver. They assess clients and their situations to make positive changes. Must maintain knowledge of current programs and services to offer practical and timely solutions. Creativity is key, because some cases can demand problem solving that involves multiple generations and differing age-specific needs. Will oversee several clients at a time, ensuring information is accurate, maintaining and keeping detailed documentation of each client. Engaging with clients from diverse backgrounds requires this position to have developed sensitivity to, and a general understanding of, a variety of cultures, religions, and social norms. Will work with patients in hospitals, shelters, assisted living facilities, clinics, group homes, and nonprofit organizations.

Responsibilities

  • Provide transport via company vehicle/van for clients to appointments
  • Daily check in with clients to identify and follow up on needs
  • Maintaining communication with hospital staff
  • Documenting all interactions with clients through shared excel spreadsheet
  • Collaborate and interact with other institutions
  • Analyze and evaluate residents upon admission and ongoing
  • Evaluate clients and identify the sort of assistance they require
  • Work with the Director of Client Services to develop treatment programs
  • Research and arrange community services
  • Rotating on call assignment

Requirements

  • High School Diploma or equivalent
  • Home care experience: 1 year (preferred)
  • Health Care Experience: 1 Year (preferred)
  • Case Management experience: 1 year (preferred)
  • Scheduling experience: 1 year (preferred)
  • Driver's License (required)
