POSITION SUMMARY:

The Adult Programs Nurse provides oversight of resident’s healthcare needs and nursing support to both the MY Life and GADOL program. Primary responsibilities include managing resident’s medical records, monitoring resident’s health status and the training and oversight of all medication administration performed by Authorized Direct Support Professionals (ADSPs).

 

 

ESSENTIAL RESPONSIBILITIES:

  • Conduct medication administration staff training. Includes 8 hours med class, annual re-evaluations, competency-based training assessments and any other related training as needed.
  • Complete nursing assessment, including evaluation of the individual’s physical status, mental status, medical history and medication orders on an annual basis or more often if needed.
  • Complete the Health Risk Screening Tool on an annual basis for each resident.
  • Assess individual’s capabilities of self-administering medication at least annually, using the SAMA assessment tool.
  • Ensure each resident’s health status has been assessed by a physician, advanced practice nurse or physician assistant at least annually.
  • Receive all medications and review labeling and orders to make sure these include all necessary information, per Rule 116.
  • Review all medication administration records for completeness and accuracy, at least on a monthly basis. This includes controlled substance count sheets.
  • Maintain an inventory and record of the use of controlled substances.
  • Conduct random medication observations of each ADSP on a quarterly basis and conduct re-training, as needed.
  • Check medications at each residence on a quarterly basis for expiration dates.
  • Review medication error reports on a monthly basis and work with Director and Vice President to develop corrective action plans.
  • Maintain knowledge and oversee implementation of all items included in Rule 116.

Qualifications

QUALIFICATIONS:

  • Requires a Registered Nurse (RN), who is licensed to work in the state of Illinois
  • Nurse Trainer certification completed by the DHS DDD.
  • At least two (2) years of experience as a nurse in a CILA facility or similar position
  • Knowledge of DHS Rule 116
  • An unblemished nursing and background record
  • An effective communicator, verbal and written
  • Experienced in medication management
  • Ability to positively interface with our clients and staff
  • Valid driver’s license and safe driving
