Position:                    Medical Services Coordinator for MY Life

Location:                    Highland Park

Supervisor:                Director of Residential Programs

Schedule:                   Varies

FLSA Classification: Full-time / Non-exempt / Salary

 

POSITION SUMMARY:

The Medical Services Coordinator will support the health and wellness of the My Life residents. This person will report directly to the Director of My Life. In the absence of a Director, the Medical Services Coordinator will report directly to the Vice President of Adult Services.

 

The Medical Services Coordinator will work during typical business hours, with occasional nights and weekends as assigned. This individual will work in the office located at 2121 as a primary work space; however, travel to all residential locations, the business office, Gadol, and within the community is required.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Medical Services:

These duties relate to the coordination and tracking of routine medical appointments. The Medical Services Coordinator will schedule all appointments, attend appointments with residents, provide the necessary forms for Quality Assurance purposes, and document the appointments using Therap. This may include medical appointments, dentist appointments, counseling and ancillary therapeutic services, adaptive equipment, etc. The Medical Services Coordinator will review all after visit summaries and disseminate information regarding these appointments to the House Manager, QIDP, Nurse Trainer, etc. Additionally, the Medical Services Coordinator will notify each individual’s support team and the Nurse Trainer regarding any acute health issues or changes in health status.

Medications:

The Medical Services Coordinator is responsible for supporting the Nurse Trainer with the monitoring and oversight of medications. At the beginning of the medication cycle, the Medical Services Coordinator will check in all cycle medications, notifying the pharmacy of any missed medications. The Medical Services Coordinator will notify each individual’s support team and the Nurse Trainer regarding any changes in medications. Additionally, the Medical Services Coordinator will request medication refills and non-cycle medications from providers as needed.

Record Management:

These duties ensure that all residents have an accurate and well-maintained health record in Therap. This will include documenting all appointments into Therap and uploading all after-visit- summaries into Therap after appointments are completed. Additionally, the Medical Services Coordinator will review all GERs associated with medical administration. Regularly scheduled audits of health records will be performed to ensure that all needs regarding compliance are met.

Health Tracking:

Because regular health tracking is critical for the well-being of residents, the Medical Services Coordinator will oversee the completion and documentation of the weight and blood pressure of all residents. Health tracking that is related to a specific individual will also be monitored by the Medical Services Coordinator, as needed.

Assessments:

The Clinical Coordinator is responsible for monitoring the timely completion of yearly nursing assessments (SAMA, HRST, Nursing assessment). In order to support the residential team, the Medical Services Coordinator will function as the primary HRST rater for Keshet.

Other Duties:

Adult Services must operate as a team in order to function. As is expected in all roles, the Medical Services Coordinator will fill in for others in the event of an absence, vacation, leave, etc. When requested, the Medical Services Coordinator will attend team meetings, house meetings, annual meetings, etc., to report on the health and wellness of residents. To support the need of residents, the department, and the agency, other duties may be assigned on a temporary or permanent basis to ensure Keshet’s ability to provide quality, person-centered services that ensure the health and wellness of its residents.

Qualifications

QUALIFICATIONS:

A bachelor’s degree

At least 1-year experience working with adults with I/DD is required Excellent

Certification as a QIDP is highly preferred

Proficient in Microsoft Office Suite, computer skills, and database experience.

Excellent communication skills in the English Language, verbally and in writing

Excellent organizational strategies

A valid driver’s license and safe driving record is required.
