Erie Family Health Centers, a nationally recognized top workplace with 13 sites in Chicago and suburbs, is looking for a valuable addition to our Health Promotions team! The Senior Coordinator, Health Promotions is responsible for health promotion and disease management program initiatives. They coordinate and implement group programming and health education visits and are responsible for the broader department disease management and prevention content and activities. The Senior Coordinator will facilitate the hiring, onboarding, and oversight of the Health Promotions team.

Main Duties & Responsibilities

Program Coordination

  • Responsible for the planning, evaluation, and efficient implementation of program initiatives, group programming, and health promotion activities.
  • In coordination with the Senior Manager, Patient Programs assure completion of grant goals, program success for relevant health promotion grants, and alignment with and support of agency and key stakeholder priorities.
  • In coordination with the Senior Manager, supports the development and revision of Standardized Operation Procedures (SOPs) and workflows to support program structure and staffing productivity

Supervision

  • Ensures staff in all programs enter data according to standards of the program to ensure goals are achieved for grants/ stakeholder requirements.
  • Supervises staff through 1:1 meeting, conducts formal evaluations, and performance improvement plans. Participates in the recruitment, interviewing and hiring of health promotion program staff.
  • Collaborates with other members of the Health Promotions Team, Patient Programs and Supportive Services Department, and broader organization to plan, implement, and support other initiatives as assigned.
  • Provides direct patient services and support when needed for role modeling, back up for their team, and coverage during vacancies or leaves.
  • Other duties as assigned.

Qualifications

Education

Required:

  • High school diploma or equivalent, with 6 years of progressively responsible leadership experience; OR
  • Associate degree or two years of college, with 4 years of progressively responsible leadership experience; OR
  • Bachelor’s degree from a 4-year accredited college or university with 2 years of progressive leadership experience.

Preferred:

  • A Master's/advanced degree, or additional credential/ certification in a relevant field (MPH, RD, CDCE, nutritionist, CHES, etc.)

 

Experience:

Required:

  • Minimum of two years of experience supervising staff and project/program coordination or management.
  • Demonstrated cultural competency to work in a low income, minority, environment.

Preferred:

  • Experience conducting individual or group education, chronic disease self-management or prevention support or overseeing health promotion services.
  • Additional supervision and program coordination experience.
  • Experience working in a community health setting.

 

Skills and Knowledge

Required:

  • Ability to establish and maintain effective working relationships with co-workers, clients, and community partners.
  • Experience with program coordination and project management, including the ability to plan, implement, evaluate, and modify programs based on assessment and needs.
  • Ability to solve problems and address issues as they arise.
  • Strong interpersonal and oral communication skills and the ability to work with staff members across all levels of the agency.
  • Strong computer skills: expert in Microsoft Office, Excel, and ability to learn to expert level other electronic databases (Epic electronic medical record system, etc.).
  • CPR certification required.

Preferred:

  • Bilingual fluency (English/Spanish) a plus. (May be required to assist or work with non-English/Spanish-only speaking patients or guests.)
  • Experience working in an Electronic Health Record System.
