Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Advanced Practice Provider Team! The Urgent Care Primary Care Advanced Practice Provider or Physician will provide comprehensive primary health care and urgent care services to patients. Performs physical examinations on new and return clients. Refers patients to specialists as necessary. During peak viral season, provides urgent care services with a special focus on pediatric patients. Conducts care of newborns, hospital discharge patients, and pre-operative patients.

Main Duties & Responsibilities
- Performs history and physical on new/return patients throughout the lifespan.
- Assess, plan, and treat patient needs within an urgent care setting and triage patients to differentiate between immediate and less urgent care.

- Order, collect and perform appropriate laboratory and diagnostic studies.
