Position Purpose

The Mill Levy Program Access Coordinator provides access to Mill Levy for eligible Denver residents. This position will primarily work with people birth through life with an intellectual/developmental disability and connecting people to other resources. This position will collaborate with case managers at RMHS to support people in getting connected to Mill Levy to support when there are no other resources to meet or fully meet the need.  These individuals may include those that receive services from another Case Management Agency and/or that do not qualify for or are on a waitlist for Medicaid or State funded programs. In addition, the position will work with members who are seeking services and are transitioning between programs.

The Access Coordinator works closely with eligible Denver residents and their families to complete individual requests and identify needs that can be met through a Mill Levy Support Plan, which offers access to qualified providers and services that mirror Medicaid Home and Community Based Waiver services.

Essential Duties

  • Provides timely and responsive customer service to eligible Denver residents seeking Mill Levy funded goods, services, and supports to benefit their life and wellbeing.
  • Completes, submits, and monitors progress of individual requests in multiple data systems
  • Completes, submits, and monitors progress of Mill Levy Support Plans for eligible Denver residents that do not qualify for Medicaid or state funded services
  • Ensures client-specific information is documented and reported timely and accurately, and documents are uploaded and stored in the Rocky Mountain Human Services data systems
  • Develops and maintains supportive working relationships with assigned Mill Levy Access Coordinators, Service Coordination department, Developmental Behavioral Health serving people with intellectual/developmental disabilities to ensure the delivery of quality services for eligible Denver residents 
  • Aids individuals, families, providers, vendors, and internal program areas throughout the individual request process.
  • Conducts outreach on an annual basis for individuals waiting for residential services and not enrolled in a Medicaid or state-funded Supported Living Services (SLS) program to offer a Mill Levy Support Plan.
  • Provides information regarding other community resources that may be available to meet individual needs.
  • Performs other duties as assigned to the Mill Levy Program operations and team functions.
  • Ability to drive personal or company vehicle

Qualifications

Knowledge, Skills, and Abilities

  • Demonstrates support for the company’s mission, vision, and values 
  • Basic skill in computer software programs including databases and on-line applications
  • Ability to analyze information and use sound judgment, problem solve, and use critical thinking skills in a fast-paced environment
  • Excellent written and verbal communication skills, strong organizational and time management skills, and strong interpersonal skills
  • Ability to maintain strict HIPPA confidentiality guidelines
  • Ability to effectively manage multiple priorities and projects
  • Ability to function as a team player and work cross departmentally
  • Ability to lift and/or carry 20 lbs / Sit, stand and walk for reasonable periods of time.

Minimum Qualifications

  • GED/high school graduation
  • 1-2 years experience with IDD population
  • Must reside in the state of Colorado – RMHS is a remote-first company and some in-person work is required
  • Ability to read, write and speak English fluently 

 

 

Preferred Qualifications

  • Previous experience in a Human Services field
