Description

Summary: This position is responsible for the intake process for the Roommate Housing Program including referrals, matching survey, events, family meetings, apartment applications, coordination of technology assessment, etc. This includes coordinating roommate matches, guiding families through the housing process, ensuring effective communication with all families and individuals served, and managing housing logistics (lease, utilities, cable, etc.).

 

Essential Duties and Responsibilities: include the following. Other duties may be assigned.

 

  • Manages and directs all aspects of the Roommate Housing interest list.
  • Schedules and coordinates tours for interested individuals, family members and other stakeholders.
  • Coordinates activities of the organization’s Roommate Housing Team.
  • Engages with referrals to determine housing needs, answer questions, and initiate the intake process.
  • Once the roommate match is confirmed, communicates with CES Waiver DSM to lead the development of the waiver plan of services for the individuals who will share an apartment and share overnight staffing and remote monitoring.
  • Maintains current knowledge of eligibility, funding, and licensure requirements (DHS/DDS, DPSQA, PASSE).
  • Maintains information in database.
  • Provides weekly reports on the status of referrals and participates in enrollment meetings.
  • Manages inventories and purchase supplies for the department.
  • Assists individuals in identifying and utilizing community resources and natural supports by working cooperatively with individuals, families, PASSE, state and federal agencies, and other service professionals.
  • Thorough knowledge of CES Waiver licensing regulations and ESA policies and procedures.
  • Complies with all policies and procedures included in ESA’s Rules of the Road defensive driving manual.

While performing the duties of this job, employee will comply with all federal, state, and local laws and regulations pertaining to Easterseals Arkansas.

 

Employee has access to all client protected health information within his/her program.      Employee will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.

 

Supervisory Responsibilities:

No direct reports.  No supervisory responsibilities.

Qualifications

Education and/or Experience:

Bachelor’s Degree preferred. High School Diploma/GED and 3-5 years’ experience in providing administrative services in a program that supports individuals with an Intellectual Developmental Disability, or other Social Services or Health-Related program.

  • Proficient knowledge of technology, and possesses ability to navigate multiple technology platforms

  • Ability to present to groups and answer questions – both in person and via video conference.
  • Strong computer skills to use email, calendar, word processor, spreadsheet, video conferencing, and presentation software.
  • Demonstrated ability to work effectively and collaboratively with individuals served who have diverse learning levels.
  • Ability to manage a diverse and demanding workload.
  • Demonstrated commitment to the values of diversity, equity, and inclusiveness

 

 

Certificates, Licenses, Registrations:

Valid Arkansas Driver's License
