Description

Summary: Responsible for the coordination of admissions and annual eligibility for individuals to receive services including completion of intake documents, assessment, funding confirmation, and coordination with other services within Easterseals and outside of Easterseals. Maintain organized, accurate, complete, and detailed individualized case file records that comply with all organizational and regulatory standards for services received.

Essential Duties and Responsibilities: include the following items, but other duties may be assigned.

Productivity and Reliability:

  • Coordinates the admission of all assigned referrals and provides assistance with procuring required documentation necessary to determine eligibility.
  • Coordinates the development of the Individual Treatment Plan (ITP) at the time of admission and annually thereafter.
  • With input from individual served, stakeholders and other team members, develops long and short-term goals and corresponding objectives that are clear, functional, measurable, and support the individual’s level of independence.
  • Reviews each ITP and goals/objectives at least quarterly to determine effectiveness and progress.
  • Maintains case files for individual’s served in compliance with all organizational and regulatory standards including utilizing electronic medical records system or other systems to track due dates for prescriptions, annual plans, reporting, etc.
  • Assists individuals in identifying and utilizing community resources and natural support networks by working collaboratively with individuals, families, state and/or federal agencies and other service professionals.
  • Works with individual served and if applicable the parent/guardian to resolve crises.
  • Utilizes positive behavior intervention techniques to support the individual in managing and sustaining behavior that allows them to engage appropriately with peers and staff.
  • Assists individuals in self-advocacy.
  • Participates in activities that promote community awareness of Easterseals Arkansas programs.

Other duties as assigned to include but not limited to and are subject to change:

  • May be required to operate an Easterseals vehicle to transport individuals served to offsite service locations
  • Attend transition fairs and transition IEP meetings at local schools to recruit applicants
  • Provide classroom and front desk coverage (as needed)
  • Provide arrival and dismissal coverage in the afternoon and/or morning (as needed)

While performing the duties of this job, employees will comply with all federal, state and local laws and regulations pertaining to Easterseals Arkansas.

Employee has access to all client protected health information within his cohort of individuals served. Employees will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.

Supervisory Responsibilities:

This position has no supervisory responsibilities.

Qualifications

Education and/or Experience:

Bachelor's degree (B. A.) preferred from a four-year college or university in Psychology, Social Work, Special Education or related field or a minimum of 5 years equivalent experience working with individuals with IDD.

High School Diploma

Valid Arkansas Driver's License

Maintain Proof of Personal Auto Insurance
