Summary: To deliver care to individuals in an ICF/IID residential setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation and in collaboration with other professional disciplines to ensure effective care delivery and achievement of desired outcomes of individuals served.
Essential Duties and Responsibilities: include the following. Other duties may be assigned.
Consults and coordinates with health care team to assess, plan, implement, and evaluate individual’s nursing care plan.
Ensures physician orders are properly documented and followed accurately.
Draws labs as ordered by physician and documents in accordance with policies and procedures.
Provides nursing care according to policies and procedures in accordance with the Nurse Practice Act and with the Office of Long Term Care regulations for ICF/IID facilities.
Prepares and administers (orally, subcutaneously, or through an IV) and records prescribed medications.
Follows procedures for counting, documenting, and storing medications, including narcotics, received from the pharmacy and from the individual’s parent/guardian upon admission and/or return to the facility.
Follows procedures for counting, documenting, and disposing of medications, including narcotics.
Documents and reports adverse reactions to medications or treatments to Medical Director or designee, DON and Director/Administrator as defined in policies and procedures.
Records and reports medication errors and incidents/accidents as soon as made aware of the error, incident, or accident.
Records an individual’s medical information, change in status, and vitals when there is an emergent change in the individual’s status. Reports emergent changes in individual’s status to the Medical Director or designee, On-Call RN, and Director/Administrator.
Completes and documents assessment of individual prior to individual leaving the facility and when the individual returns to the facility and when there is an incident/accident involving the individual.
Accurately charts response to plan of care and care given in the medical record including monthly vitals of individual. Maintains the medical record of individual served.
Assists Habilitation staff, as needed, in providing personal care (changing, bathing, feeding, etc.) to individuals.
Assists, as needed, in transporting (driving ESA van or riding with individual) individuals for clinic appointments and hospitalizations.
Adheres to and enforces facility infection control policies and procedures.
Protects and promotes rights of individuals served.
Communicates with families of individuals served, other staff, physicians, pharmacies, other facilities, and support organizations in person and by telephone as individuals' needs and condition warrants.
Reports concerns that may significantly impact care of individuals served, safety and security to the DON.
Provides nursing functions at Butler Adult Living Center as assigned.

Employee has access to all client protected health information within his/her program. Employee will not attempt to access any other protected health information unless otherwise approved by Supervisor or Privacy Officer.
