Employment Type: Full-Time, Hybrid (Up to 2 remote days per week)
Pay Rate & Type: $23.54/hr

Job Summary

Provides basic consultation and support services to trust beneficiaries and their families. Through direct contact with beneficiaries, advocates for and educates regarding support plans that maximize all resources available to the person. Meets with potential beneficiaries, families, and attorneys to provide information on special needs trusts. Makes presentations to interested groups to provide information on special needs trusts.

Key Responsibilities

  • Supports the achievement of individuals personal outcomes.
  • Meets face to face and maintains contact with trust beneficiaries and others involved in the individual's life, including family, professional staff, and others.
  • Evaluates the beneficiary's need for support services, including current services being provided through government funding and those that can be provided through the use of funds in the trust.
  • Works collaboratively with staff and family supporting the individual to develop a support plan that maximizes government benefits and services funded through the trust to enhance the quality of life for the beneficiary.
  • Attends regularly scheduled planning meetings for the beneficiary conducted by support agencies.
  • Identifies community resources that can provide contracted services with funds in the trust.
  • Arranges for appropriate support services to be paid for through the use of funds in trust.
  • Reviews requests for disbursements for beneficiaries from funds in trust. Assures accuracy and appropriateness of all fund requests submitted.
  • Monitors services being provided by trust funds to assure that they meet the beneficiary's needs and client satisfaction. and maintain files and written case notes for trust beneficiaries.
  • Meets with attorneys, family members, and individuals to provide trust information to assist individuals interested in creating a special needs trust.
  • Speaks to groups as requested and attends vendor fairs to provide information on special needs trusts and to promote the services of Achieva Family Trust.
  • Maintains awareness of new developments and current trends in area(s) of expertise.
  • Participates on various work teams as requested or appropriate.
  • Performs other related duties as assigned. Qualifications

  • The equivalent of a BS or BA in social sciences, counseling, or related degree.
  • At least 1 year of experience in social services (case management, service coordination, etc.
  • Knowledge of services in mental health, intellectual disabilities and/or physical disabilities.
  • Demonstrated ability to understand and comply with financial and legal concepts
  • Ability to complete Act 33/34 background checks upon employment.
  • Valid drivers license and properly insured vehicle for daily use.
