Position Summary: Provide information to outside sources on WellLife Network programs and availability.
Essential Accountabilities:

Handle telephone inquiries requesting information about WellLife Network programs; determine the type of service that is being requested and refer to appropriate internal source or appropriate outside agency.

Outreach to various state and local entities as warranted to present WellLife Network Housing Services.

Point of contact for accessing all HRA/CAPS referrals (i.e. coordinating schedules with Intake Coordinators to establish screenings for each referral received). Update information as warranted, as back up to Intake Coordinators.

Responsible for all data entry into AVATAR system, including all preadmissions for new referrals, documentation uploads, etc.

Schedule site visits as warranted.

Maintain a resource list for Intake (pharmacies, PROS programs, Care Coordination, Contacts, etc).

Maintain point of contact for all emails received to Residential Services email address.

Responsible for obtaining all program census monthly for all beds under MH Housing.

Maintain an internal tracker for all admissions and discharges.

Attend meetings as determined by management.

Position requires some field work (vehicle required) and 2 days weekly in office. Other days may work remote or as determined by Director of Intake Services.
Other Responsibilities:

Make community contacts with possible referral sources, clinics, in or out patient departments, psychiatrist, day programs, etc.

Maintain confidentiality at all times.
