Position Summary: Supervises and coordinates the work of case managers, engaged in providing enhanced ADL (Activities for Daily Living) skills to a resident population of apartment program consumers. Ensures compliance with all appropriate laws and regulations. This position may have supervised / unsupervised contact and or conversational interaction with consumers while working in the residences, day treatments and habilitation facilities
Essential Accountabilities:
- To ensure delivery of service in a safe and therapeutic manner; in order to help consumer to move forward in their recovery. Minimize risk and reduce crisis situations.
- Provide Clinical supervision and coordinates the activities of Case managers.
- Communicating Agency plans, programs, and policies; and ensuring the consistent application of Agency policies and procedures.
- Coordinating, and reviewing work to promote efficient operations in accordance with OMH regulations
- Interviewing, selecting, hiring and orienting new staff; providing job-specific training regarding assigned tasks; conducting annual performance appraisals; counseling on personal development needs of staff and developing action plans.
- Acts as liaison between consumer and landlord. Visits consumers’ homes a minimum monthly to monitor delivery of services, ensuring a safe and well-maintained environment.
- Secure, coordinate, and monitor services delivered by vendors providing clinical, medical, dental, psychological/psychiatric, and/or vocational services, as well as attendance at a variety of day programs.
- Provide ongoing support in the following areas: Money management, Medication management, Daily living skills, Community integration, Maintenance of sobriety, Symptom management.
- Develops, promotes, and coordinates recreational/social programs and activities.
- Ensures the accurate and timely completion of all area documentation, including but not limited to: consumer records, time cards, incident reports, and agency reports. Audits charts routinely to ensure compliance with PSCH, OMH, and Medicaid standards.
- Liaisons with Intake and Fiscal departments, on behalf of consumers.
- Participates in a variety of meetings (e.g., area, management, supervision) and on various committees, as deemed appropriate by supervisor.
- Attend Community Meetings with consumers.
