Salary:  $$50.00 to $55.00 Per Hour Depending on Experience



MHM Services, a Centurion company, is proud to be the provider of mental health services to the California Department of State Hospitals-Forensic Assertive Community Treatment Program (FACT).  The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services.   Services needed to support community reintegration include but are not limited to the following:

  • Forensic mental health treatment through individual and group therapy settings
  • Life skills training
  • Day socialization groups
  • Collateral contacts
  • Home visits
  • Substance abuse screenings
  • Psychiatric services
  • Case Management
  • Court reports
  • Psychological assessments


We are currently seeking a full-time Mental Health Registered Nurse with MHM/Centurion at our Forensic Assertive Community Treatment Program (FACT) in Vallejo, California.  



Under the direction of the Program Director, Mental Health Registered Nurse will provide nursing care to patients in a forensic community setting to meet service delivery requirements of the FACT Collaborate with a multidisciplinary team to identify and respond to a wide range of mental and physical health needs.



Essential Duties/Responsibilities Include:  

  • Provides nursing services to individuals who have been found incompetent to stand trial, not guilty by reason of insanity, or are offenders diagnosed with a serious mental illness and/or co-occurring disorders (serious mental illness and substance use disorder(s)).
  • Collaborates with multidisciplinary team to provide necessary psychiatric nursing care to patient in the community and housing facility.
  • Contributes to multidisciplinary individualized treatment plans, collaboratively with other treatment providers and the participating patient.
  • Administer ordered medications and implement relevant treatments. Evaluate, monitor and document results/outcomes of mediations and treatments
  • Monitor and document medication administration and treatment implementation as directed at time of administration.
  • Acquire and maintain current clinical knowledge and skills and professional standards.
  • Participate in the monitoring, requisition, and documentation of the use of medical supplies and equipment as directed
  • Stores medications in a safe and accurate manner and monitors medications to ensure adequate accountability.
  • Participates in treatment team meetings; have an understanding of the importance of interdisciplinary care and how to engage in this process.
  • Provides direct and indirect nursing care to meet physiological, psychosocial, and emotional needs of the patient.
  • Implements and evaluates patient education offerings and activities; provides education and activities for patients.
  • Proactive in performance improvement activities as demonstrated by participating in the Continuous Quality Improvement (CQI) program (peer review; record reviews; membership on the CQI committee as requested) and modifying clinical practices based on CQI recommendations.
  • Ensures compliance with all facility/DSH/house and Company policies, Federal and State laws, regulations, and guidelines.
  • Establishes and maintains effective working relationships with the Judiciary, District Attorneys, Public Defenders, Parole Agents, DSH staff, and local law enforcement officials.
  • Establishes and maintains effective working relationships with the community agencies that could positively impact community opportunities for patients.
  • Identifies and maintains professional boundaries to preserve the integrity of the therapeutic process and relationship.
  • Performs other duties as assigned.


Schedule: 

  • Tuesday through Saturday 8:00am to 4:30pm

Qualifications

  • Bachelor’s degree in Nursing from an accredited institution required.
  • Must hold valid unencumbered registered nurse license in California. 
  • Minimum of 2 years’ experience working as a nurse in a psychiatric hospital, a behavioral health (inpatient or outpatient) center, correctional facility or a community behavioral health center preferred.
  • Knowledge of mental illness and co-occurring-disorders; a basic understanding of the criminal-legal system preferred.
  • Ability to plan, organize, develop, and document services in a manner that maximizes accountability; strong written, oral, and interpersonal communication skills.
  • Must be able to work independently, set and meet deadlines, and demonstrate productivity.
  • Ability to interact well with varied levels of managerial, clinical, court, and administrative staff in community environments.
  • A valid automobile driver’s license, appropriate automobile insurance, and a safe, reliable, registered automobile available for use during work is required.
  • Active in-person CPR certification required along with current tuberculosis screening.
  • Must be able to pass background investigation and obtain agency security clearance as required by DSH.
