Primary Function:
Under the direction of a Registered Nurse or Therapist, the Home Health Aide assists clients with personal care activities of daily living. The HHA reports to the HHA Nurse Supervisor.
Duties & Responsibilities:
Personal Care:
- Helps patient with bath, care of mouth, skin and hair
- Helps patients to bathroom or in using bedpan
- Helps with ambulating, i.e., transfer from bed, use of wheelchair, walker, etc.
- Helps patient with prescribed exercise, which the patient and appropriate professional personnel have taught Home Health Aide.
- Prevents deformities through body alignment in bed or out of bed, helps patient with prescribed exercises.
- Demonstrates the ability to perform selected activities once supervised by the RN/therapist including, ostomy bag and skin care, reinforce a dressing, change a non-sterile dressing, ambulation with a device, assist with prescribed skin care, perform urine sugar tests, measure intake and output
- Provides emotional stimulation and support.
Home Management: Performs such incidental household services as are essential to the patient’s health care at home.
- Caring for the patient’s environment, i.e., light personal laundry, and light cleaning.
- Teaching management skills to family members
- Promoting and assuring a safe environment
Team Member Functions:
- Reports to the nurse or therapist changes in the patient’s condition or family situation
- Attends case conferences as requested
- Required to attend minimum of 12 hours of in-service programs per year
