RN On Call - Weekends

We are hiring a full-time Registered Nurse to join our growing team in the Rock Hill, SC and surrounding areas by providing Case Management services to patients in their homes, assisted living and long-term care facilities who are enrolled in our hospice program.

***This is a weekend on call position working Friday 4:00 pm- Monday 8:30 am ***

Responsibilities:

• Looking for someone with a Hospice heart and passion for providing end of life care
• Provide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG
• Responsible for overseeing patient plan of care with the collaborating physician
• Will conduct start of care visits and collaborate with Care Navigators
• Provide education to patients, families, and facility staff

Qualifications:

• Must have a RN license unincumbered in the state you are applying
• Reliable transportation and valid drivers license, clean MVR
• Experience in Hospice/Home Health is preferred
