Advanced Practice Provider (APP) – Emergency & Inpatient Care – Haxtun, Colorado
Haxtun Health is seeking an experienced Advanced Practice Provider (Nurse Practitioner or Physician Assistant) with strong Emergency Room and Inpatient care experience to join our medical team. This role is integral to providing high-quality care in our 25-bed Critical Access Hospital, with a focus on Emergency Department and Inpatient services. Haxtun Health’s typical ER volume is 500 visits per year, offering a manageable case load and ample time for patient care and interaction.
The Haxtun Health ER is supported by the Avel E-Care System, providing trained Emergency Physicians for virtual backup, along with an incredible and well-trained nursing staff for exceptional support. The hospital consists of 20 Long-Term Care beds and 5 Inpatient/Swing Bed beds. This position will manage acute medical issues for long-term care residents and work closely with our robust In-Patient Rehab program, which is supported by a highly skilled rehabilitation team, nursing staff, and care coordinator.
The APP will work as part of the entire medical team and in close collaboration with our primary care family providers to ensure local continuity of care.

Ideal Candidate:
- Current Nurse Practitioner (NP) or Physician Assistant (PA-C) certification with licensure in the state of Colorado (or eligibility to obtain licensure).
- Minimum of 2 years of experience in Emergency Medicine and/or Inpatient Care is strongly preferred.
- BLS, ACLS, PALS, and ATLS certifications required (or willingness to obtain upon hire).
- Strong clinical decision-making skills with the ability to work independently and confidently in a Critical Access Hospital setting.
- Experience with Swing Bed programs and/or Long-Term Care patient management is a plus.
- Excellent interpersonal and communication skills, with the ability to collaborate effectively across all levels of the healthcare team.
