BASIC PURPOSE: To provide nursing services to assist persons with severe and persistent mental illness in the CSP program to live successfully in the community and grow their mental health recovery. To provide assistance to clients particularly related to health care and other medical illnesses/problems they experience, to help clients with their medical health needs.

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Provide direct nursing services to clients of the CSP program, including medication monitoring, medication administration (including injections), and medication education. Provide screening for medication side effects and tardive dyskinesia, as required and in coordination with the psychiatrist.
  • Attend CSP team meetings to coordinate care related to client health and medications. Provide medical expertise and consultation to the CSP team to assist the team in providing services that best assist clients with their medical needs.
  • Coordinate with the CSP psychiatrist and other medical professionals and pharmacies to assist clients in attaining the medications they need for both their mental health and general health. Provide preparation or coordination of Prior Authorizations for medications, as needed. Coordinate client lab work needed and lab work results, in coordination with the CSP psychiatrist.
  • Provide direct nursing services to clients of the CSP program to provide basic medical care, such as blood pressure screening, BMI, weight, heart rate monitoring, blood sugar monitoring and foot checks and care. Provide health education and assistance to clients in understanding medical diagnoses, procedures, health recommendations of doctors and other health professionals, and health practices for wellness, such as dietary changes, smoking cessation, etc.
  • Complete all required paperwork in a thorough and timely manner.
  • The nurse is supervised administratively by the clinical coordinator and clinically by the CSP psychiatrist.

Qualifications

ESSENTIAL KNOWLEDGE, SKILLS & ENVIRONMENT:

Education, Training & Work Experience: Registered Nurse licensure required, with preference for experience in working with persons who experience severe and persistent mental illness.

Specialized Knowledge & Skills: Familiarity with mental health diagnoses, medication administration and monitoring, other clinical nursing skills, and appropriate ethics and boundaries

Equipment & Applications: Ability to use Microsoft Office programs, e-mail, scanner, and FAX machine, as well as enterprise software. Driver’s license and ability to use own vehicle to provide client transportation

Work Environment & Physical Demands: Indoor office environment, with willingness to see clients in a community setting or in client’s homes. Local travel will be required. Additional physical demands could include lifting, bending, twisting, and reaching.
