Accurate Home Care, LLC is growing in the area of Home Health. We are seeking a  Physical Therapist Assistant to support patient centered care.  This position carries out effective treatment plans for patients.

Pay & Perks:

Essential Job Duties and Responsibilities:

  • Provide patient centered treatment visits in their individual homes, or within a Senior Living Community
  • Implement client treatment plans with guidance from the physical therapist
  • Instruct patients on physical therapy interventions, modalities and home exercise programs
  • Deliver treatments/interventions for patients that allow for exceptional patient care
  • Understand and comply with policy and procedures set forth by Accurate Home Care
  • Maintain compliance with county, state and federal regulations affecting home health care
  • Must be able to commute within the Twin Cities and surrounding Metro Area
  • A valid Driver's License and reliable vehicle are required
  • Additional tasks, as assigned by manager

Qualifications

Education Requirements:

· Current Physical Therapist Assistant / PTA license in the State of Minnesota

Continuing Education Requirements:

· Complete education annually, as required by governing bodies, and/or company policies and procedures.

Additional Requirements:

· Ability to read, write and speak fluently in English.

· In the last five (5) years, two (2) years of experience as a PTA that is relevant to the patient population assigned is preferred.

· Working knowledge of Microsoft Office suite.

· Excellent oral and written communication skills.

· Ability to read, write and speak fluently in English.
