Position Summary
Are you a Registered Nurse (RN) looking to provide 1:1 care, manage your own schedule, and grow your skills? Accurate Home Care, and its Home Health service line, is growing and adding a RN Admission/ Visit Nurse to its team.
RN Admission/ Visit Nurses serve patients within an approximate 60-mile radius of our Otsego office. We always strive to keep nurse territories as condensed and close to home as possible.

What will I be doing?
- Assure appropriate care of patient is provided through admission assessments, care planning, goal setting, evaluation, coordination of care, and emergency/safety plans.
- Complete OASIS assessments at prescribed time points including but not limited to Admission/Start of Care.
- Identify potential referrals for services from other disciplines that have not yet been ordered by the physician.
- Collaborate with clients, families, physicians, and other members of the patient care team.
- Assess and monitor patient, maintaining current documentation in clinical record; reporting concerns to the Director of Home Health.
- Provide instruction to patient and family on illness and treatment regime and proper health maintenance.
- Administer medications and treatments as prescribed by the physician within scope of practice and according to the Company’s policy and procedure.
